Details of Service / The development of Integrated Care is a major element of Health and Social Care reform in Ireland. Clinical Strategy Programmes Division (CSPD) strategic role is to develop a national, strategic and co-ordinated approach for the design of clinical service improvements to deliver improved patient care, improved access and better use of resources. To this end, the CSPD is positioned as the Clinical Design Authority for reform with responsibility for directing that changes implemented by the five operating Divisions are consistent with models of Integrated Care. The CSPD is forming five Integrated Care Programmes (ICP’s) to support a fundamental requirement of Future Health, to deliver better, more integrated and responsive services to people in the most appropriate setting. The first phase of National Clinical Programmes has been based around developing excellence in individual specialties, specific diseases and stages of care. The Integrated Care Programmes represent the next stage and will be addressing where these individual programmes need to be integrated, to provide a more effective end-to-end patient journey, particularly where patient needs are complex and involve multiple encounters delivered across a range of providers. The Integrated Care Programme for Patient Flow has been identified as a priority ICP.

This role presents a significant opportunity to enable through design the delivery of Integrated Care which is seen as key to the reform of the Health Services. It will build on the Service reforms of Hospital Groups and Community Healthcare Organisation establishment to improve the quality and outcomes of care for our service users.
Purpose of the Role / The Senior Programme Manager will executively lead and manage the Integrated Care Programme. The goal is to design, agree and document integrated care pathways framework to incorporate cross service delivery, multi -disciplinary care and support which will facilitate the maintenance of health and the delivery of appropriate high quality evidenced based care delivered in a co-ordinated manner which feels seamless to the user
Reporting Relationship / The post holder will report directly to the General Manager, Portfolio Management Office, CSPD. A close working relationship with the Integrated Care Programme National Working GroupExecutive Chair,National Clinical Advisors, Group Clinical Leads, the CSPD senior management team and the System Reform Group Business Partner is essential to ensure the effective progress and management of the Integrated Care Programme.
Duties & Responsibilities / •Provide leadership and direction to the Integrated Care Programme to enable and manage the realisation of the programmes vision, key milestones and deliverables
•Facilitate the development of programme initiation and planning documents and ensure the overall programme management approach is in line with agreed System Reform Programme methodologies and other National Policies and Procedures
•Coordinate and allocate programme resources across the integrated care programme; bring drive to the programme; co-ordinate existing programme resources and capacity; develop a structured and resourced plan to deliver the programme; deliver programme monitoring and reportingto capture the evidence that will demonstrate success.
•Work within the Integrated Care and overall reform governance structures to ensure that this programme of work aligns to and enables other organisational reform programmes and strategic priorities
•Work with the GM, PMO to ensure appropriate Governance is in place at all levels of the Integrated Care Programme.
•Work with the Integrated Care Programme National Working Group, its Sub Groups and other Advisory Groups to develop a framework for the management and delivery of health services which ensure that patients receive a continuum for preventative, diagnostic, care and support services, according to their needs over time and across different levels of the health system. The supporting Models of Care will incorporate cross service, multi-disciplinary care and support which will facilitate the maintenance of health and the delivery of appropriate high quality, evidence based care, delivered in a co-ordinated manner which feels seamless to the user.
•Design, plan and agree workstream programmes with clearly defined objectives and milestones which provide a framework to deliver the outputs and outcomes required to enable the agreed benefits to be realised for the integrated care programme.
•Coordinate the activities and outputs of the Integrated Care Programme Team and its workstream groups.
•Work collaboratively with HSE operating division National Clinical Advisor and Group Leads (NCAGL’s) and their programme managers to ensure there is a clear understanding of the work being pursued by all parties in relation to National Clinical Programmes and/or Integrated Care Programmes.
•Develop and give effect to the benefits realisation plan in conjunction with key stakeholders
•Develop and manage stakeholder engagement and communication plans
•Organise and facilitate stakeholder engagements
•Plan, prepare and manage the programme reporting, for the Programme working group and other stakeholders.
•Facilitate the programme team in agreeing implementation strategies, plans and conducting change impact assessments
•Oversee the establishment of demonstrator projects to inform, improve and have proof of concept that the designed and agreed integrated model of care framework will deliver the agreed benefits if implemented correctly.
•Develop good collaborative working relationships with the National Clinical Advisor Group Leads, Clinical Leads, Programme managers and key stakeholders. Meet with Programme Managers and/or Clinical leads with the responsibility for executively leading and managing programmes within the Integrated Care Programme on a regular basis to discuss and update on the status/progress versus the plans and provide support in relation to issues arising.
•Work with CSPD PMO and Senior programme management team to develop standards of practice, standard operating model frameworks, tools, documents and systems.
•Understand the external communication needs of the CSP Division, build processes and source contacts to ensure all records and reports are in the format required, are readily available to assist with the provision of the information.
•Support the CSPD in relation to ensuring the provision of national strategic input to the yearly national service planning process including setting national priorities and key performance indicators and financial planning.
•Work with the National Director and CSPD Team to improve capacity within the Division for issue identification, national policy prioritisation, risk assessment and future planning.
•Contribute to the ongoing development and improvement of the operations of the Division. Ensure that the knowledge and learning gained within the Integrated Care Programmes is captured and incorporated in future materials in order to maximise impact and sustainability of the programmes to embed continuous improvement.
•Assist the CSPD team to plan, measure and record its performance in line with the CSPD and HSE corporate reporting requirements. Assist in the identification and development of Key Performance Indicators for the Integrated Care programme. Report on agreed KPI’s to the programme office in accordance with the agreed reporting and communication protocols.

Grade VIII

Terms and Conditions of Employment

Tenure / The post is permanent and whole time. The post is pensionable. A panel may be created from which permanent and specified purpose vacancies of full or part time duration will be filled. The tenure of these posts will be indicated at “expression of interest” stage.
Appointment as an employee of the Health Service Executive is governed by the Health Act 2004 and the Public Service Management (Recruitment and Appointment) Act 2004.
Remuneration / The Salary scale for the post is: €64,812 - €74.551 (July 2013)
This rate takes account of the reduction provided for by the Financial Emergency Measures in the Public Interest Act 2013.
Working Week / The standard working week applying to the post is 37 hours net per week.
HSE Circular 003-2009 “Matching Working Patterns to Service Needs (Extended Working Day / Week Arrangements); Framework for Implementation of Clause 30.4 of Towards 2016” applies. Under the terms of this circular, all new entrants and staff appointed to promotional posts from Dec 16th 2008 will be required to work agreed roster / on call arrangements as advised by their line manager. Contracted hours of work are liable to change between the hours of 8am-8pm over seven days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement (Implementation of Clause 30.4 of Towards 2016).
Annual Leave / The annual leave associated with the post is to be notified at job offer stage
Superannuation / This is a pensionable position with the HSE. The successful candidate will upon appointment become a member of the appropriate pension scheme. Pension scheme membership will be notified within the contract of employment. Members of pre-existing pension schemes who transferred to the HSE on the 01st January 2005 pursuant to Section 60 of the Health Act 2004 are entitled to superannuation benefit terms under the HSE Scheme which are no less favourable to those which they were entitled to at 31st December 2004
Probation / Every appointment of a person who is not already a permanent officer of the Health Service Executive or of a Local Authority shall be subject to a probationary period of 12 months as stipulated in the Department of Health Circular No.10/71.
Protection of Persons Reporting Child Abuse Act 1998 / As this post is one of those designated under the Protection of Persons Reporting Child Abuse Act 1998, appointment to this post appoints one as a designated officer in accordance with Section 2 of the Act. You will remain a designated officer for the duration of your appointment to your current post or for the duration of your appointment to such other post as is included in the categories specified in the Ministerial Direction. Ethics in Public Office 1995 and 2001
Positions remunerated at or above the minimum point of the Grade VIII salary scale / Positions remunerated at or above the minimum point of the Grade VIII salary scale (€ 64,812 as at 01.01.2010) are designated positions under Section 18 of the Ethics in Public Office Act 1995. Any person appointed to a designated position must comply with the requirements of the Ethics in Public Office Acts 1995 and 2001 as outlined below;
A) In accordance with Section 18 of the Ethics in Public Office Act 1995, a person holding such a post is required to prepare and furnish an annual statement of any interests which could materially influence the performance of the official functions of the post. This annual statement of interest should be submitted to the Chief Executive Officer not later than 31st January in the following year.
B) In addition to the annual statement, a person holding such a post is required, whenever they are performing a function as an employee of the HSE and have actual knowledge, or a connected person, has a material interest in a matter to which the function relates, provide at the time a statement of the facts of that interest. A person holding such a post should provide such statement to the Chief Executive Officer. The function in question cannot be performed unless there are compelling reasons to do so and, if this is the case, those compelling reasons must be stated in writing and must be provided to the Chief Executive Officer.
C) A person holding such a post is required under the Ethics in Public Office Acts 1995 and 2001 to act in accordance with any guidelines or advice published or given by the Standards in Public Office Commission.
