Applications must be received by the by OSU Extension – Southeast Region Office. E-mail proposals are preferred ( and cc: to ) but a faxed copy is acceptable (740-732-5992). Mail via postal service must be received by deadline. No late applications will be accepted.


Please answer each of the following questions in order to be considered (If applicable)

Section A – Program Support Narrative maximum of two pages plus budget page in a 12 point font.

1.  Describe the project for which you are seeking funding. Include your goals and objectives as well as the plan for achieving them. Please be sure to indicate how this project impacts the OSU Extension, the Southeast Region and/or your EERA. Include timeline information. Provide your plan for evaluating the project.

2.  What resource materials (if any) will be created that can be shared throughout the state?

3.  Describe the level of need in your community for the project you have selected. Please be sure to include information about the target audience to be served. (ie. how many individuals will benefit, ages, gender, etc.)

4.  A final report will be due one month after the completion of the project. (Project deadline is no later than December 31 of program year for which funds were awarded.)

5.  Attach a detailed project budget including income sources and expenditures. Also, where appropriate, identify other current or potential sources of support for this project. Projects that have a one-to-one match may be given extra points in the evaluation process but a match is not required.

6.  How will you sustain programming after the Endowment Fund project ends? Please share plans for fundraising, engaging volunteers, partnerships, etc. that will contribute to the sustainability of your program.

7.  If funding has been provided from this source in previous years, provide the grant titles, years and amount funded, objectives and one paragraph briefly summarizing each year.
