PART ONE: APPLICANT INFORMATION
Legal name and full address of home energy audit survey program. This name and address will appear on the Certificate of Accreditation and the national register of accredited home energy survey providers that is used by utilities, government agencies, and sponsoring programs.
There is a one time application fee of $500 for accredited rating providers to become accredited as a home energy survey provider.

PART TWO: CRITERIA FOR ACREDITATION
1. Please submit your standardized survey form as required in 704.1.2.5.1
2. Please provide a copy of your organization Minimum In-Home Energy Survey Report as required in 704.1.2.6
3. Please describe your process for compliance with RESNET Standard section 703.1.3.1.1 – Performance evaluation of ability to perform accurate Home Energy Surveys in accordance with RESNET Standards sections 704. Please be as specific as possible.
4. Please describe your process for meeting RESNET Standard section 703.1.3.1.2 – Continuing Education.
5. Please describe your process for meeting RESNET Standard section 703.1.3.1.3 – Home Energy Survey Professional Testing.
6. Please describe your process for meeting RESNET Standard 703.1.3.1.4 -Recertification of Home Energy Survey Professionals.
7. Please attach an unexecuted copy of your Home Energy Survey Professional written agreement to this application. Agreements must at a minimum meet the requirements of RESNET Standards section 703.1.3.1.5. Please note that the RESNET Code of Ethics must be attached to the agreement.
8. Please attach your organization’s Policies and Procedures Manual. The manual must include at a minimum the requirements described in RESNET Standard section 703.1.4.
· The Policies and Procedures Manual must include a detailed description of the process, procedures and reports required for a home energy survey professional to complete the In-Home Energy Survey.
· The Policy and Procedures Manual must address how your organization will determine and document that potential home energy survey professional candidates have the minimum skills required to conduct the In-Home Energy Survey.
9. Please attach your written home energy survey discipline procedures that include the requirements described in RESNET Standard 703.1.4.2. (Do not simply refer to your Policy and Procedures Manual when answering this question).
10. Please describe your Home Energy Survey Quality Control Process that at a minimum, includes all provisions described in RESNET Standard 703.1.4.3 (Do not simply refer to your Policy and Procedures Manual when answering this question.)
Also include the following information:
· The documentation of the qualifications of your quality assurance designee.
· A written, signed letter of intention between your organization and your quality assurance designee that a formal relationship will be entered into if this application for accreditation is approved.
11. Please provide a template of the Home Energy Survey Professional registry your organization will be using. The template does not need to include any survey professional information at this time.
12. Please provide your process for meeting the requirements of RESNET Standards section 703.1.4.5 – Complaint Response Process. Please describe how you will ensure that Home Energy Survey Professionals inform all clients about this process and how you as a provider can verify that this communication has transpired. (Do not simply refer to your Policy and Procedures Manual when answering this question).

· Have the individual or individuals applying for this accreditation ever been revoked under the name of this organization or any other organization accredited by RESNET? If yes, please explain and submit documentation regarding the outcome of the violation(s).
· Has any one associated with your organization or anyone associated with your company, including officers or certified raters, ever been suspended or revoked while performing as a RESNET certified rater or serving under another RESNET Providership, for violations of the RESNET Standards (including RESNET’s quality assurance standards) or any other violations of RESNET’s rating procedures, by either RESNET or a RESNET Rater Provider?
