SCHOLARSHIP APPLICATION FORM

This Scholarship Opportunity is being offered to public school students served by the Westlake Village Library through the generous donation of the Friends of the Westlake Village Library and endowments by it former members.

This opportunity offers a $1,000 scholarship to one graduating senior from each of the following public schools: Agoura High School, Oak Park High School and Westlake High School. Students should possess at least a 3.5 grade point average and show participation in school and/or community activities. Committees of The Friends of The Westlake Village Library will review the applications.

Copy of your official transcript of grades from all high schools attended

One Letter of Recommendation directed specifically to the Friends of the Westlake Village Library from a high school teacher or your high school counselor.

A Brief Essay of no more than 500 words describing the following:

*Do you think libraries are still relevant in this age of social media?

*How can libraries improve or be more involved with learning in their communities?

*Write about your personal, educational and career goals. Will a library help you with these?

*Discuss your reason for applying and include financial need if any.

*List on the application form extra-curricular activities, community involvement and employment history, if any.

The Board of the Friends of the Westlake Village Library (“Friends”) shall determine the time and conditions of payment of scholarships at its sole discretion.
