Work Term Report Form

Using the template provided on the next pages, please prepare a report reflecting on your current work term, and planning your next steps to build on this experience. The audience for this report will be a Co-op Coordinator, faculty members and future students.

Assessment Criteria:

·  Identification section with all required information

·  All questions answered

·  Informative responses (useful to future students, informative for faculty)

·  Good grammar and spelling (few to no errors, meaning must be clear)

Requirements:

1.  Fill out the identification section at the top of page 1, including student name, work term # (1st 2nd or 3rd), organization, date submitted, and co-op coordinator name.

2.  Save this completed form using the file name: Lastname_Firstname.WTR and your updated resume (if you choose to attach your resume instead of a link to your LinkedIn Profile) as a Word or PDF file and submit your file in CourseSpaces in your Co-op Work Term Course – see the assignment in the “At the End of your Work Term” module.

3.  If this is your 3rd (or last) work term, please fill out the questions on the last page about your job status and future career search plans.

9. Attach your updated resume, including this co-op work term and save form and resume together as one Word file.
