Summer Program Parent and Student Requirements
Please read these Program requirements carefully. Check each statement after you have read it. Sign and return this acknowledgement with the Registration form no later than May 12, 2018.
Family must be registered as Parishioners of Saint Leo the Great prior to 3/30/2018.
Students must attend all sessions. No student absences are allowed during the Summer Program or from the four school-year Sunday Family Catechesis sessions. Sunday Family Catechesis session dates are: October 14, 2018; December 9, 2018; January 27, 2019 and March 24, 2019.
Parents/guardians must attend all four (4) Sunday Family Catechesis sessions with their child(ren).
Late arrivals and early dismissals are not permitted for the duration of the program.
One parent/guardian from each family must volunteer a minimum of two (2) hours either during the two-week Summer session or for school-year Sunday Family Catechesis sessions. Indicate your preference for session, dates and volunteer opportunity in the box below.

All students must attend Grade level retreatsand class Masses as scheduled during the school year.
Participating families are expected to attend Mass regularly on Sundays and Holy Days of Obligation.
By signing below, I agree to adhere to all the above Program Requirements and understand that failure to do so may result in mychild(ren) being dismissed from the Program. I further understand that there will be no refunds issued for any child(ren) dismissed for rules violations.
