Boston IVF and The Surgery Center of Waltham have joined the One Medical Passport system to offer you additional convenience preparing for your scheduled procedure. Every patient that is scheduled for surgery must have a preoperative or “preop” visit. During this visit we will collect your demographic and insurance information and our nurses and/or anesthesia staff will interview you about your medical history. This information is then used to give you the best and safest care possible before, during, and after your procedure.

It is now possible to greatly expedite your preoperative visit by using the secure One Medical Passport website to complete your preoperative interview online. This can be done from the comfort and privacy of your own home or anywhere else that you have Internet access. This allows the doctors and nurses taking care of you to have all of your important medical information available in advance and will save you significant time before, during and after your surgery.

For most patients, the online interview takes approximately 20 minutes. If you ever have to return for another procedure, you only have to spend a minute or two to update your previous “pre-op”, rather than completing an entirely new pre-op visit.

Getting started on creating your Medical Passport is easy. Simply follow the steps below. We recommend that you have your insurance information and the exact names and dosages of any medications that you take available, as well as the dates of any previous surgeries.

We strongly encourage all our patients to create a

Medical Passport as soon as their procedure is scheduled!

1.  Type www.OneMP.com into your Internet browser’s address window.

2.  Click on the “Start” button on the homepage and then the “Register” button on the second page.

3.  When asked, choose the option to submit your medical information to a “Medical Facility & Physician”. The facility number for Boston IVF/The Surgery Center of Waltham is 000002.

4.  After you complete your basic medical history (8 screens), you will be asked to enter your doctor’s ID Number from the list below.

**Patients who are scheduled for an egg retrieval should list the procedure name as “VOR” (Vaginal oocyte retrieval) unless they have been specifically told that they are having a “GIFT” procedure.**
