Request to Amend or End User Login
To be completed by an Authorised Signatory*. Please print or type throughout the document, except where a signature is required. This form is to be used when the Access Rights for a person have changed (e.g. moved Department, left the Directorate, or change of Profile, Status or ICS Relationships). Personal information, including contact details, job title, etc. can be amended by the User via the Update My Profile link in the Worktray.

DETAILS OF REQUEST
The user has left the Department and will no longer be using LCS
LCS User Support will make the person's LCSl account "No Longer Active".
The user has moved Departments, but will still be using LCS
A "New User" form must be submitted by the new teambefore access will be granted. LCS User Support will remove the person's current ICS Relationships and make their LCS account "No Longer Active" until a request for a new user login is received from their new team.

AUTHORISATION
Prior to accessing IT systems and personal/sensitive information, all staff have a responsibility to ensure compliance with the Corporate IT Security Policy and any other supporting policies and procedures, which have been adopted to safeguard such information. This includes ensuring all users are sufficiently trained on the system before being given access – the user must have attended/received LCS Introductory Training (or LCS Read-Only Training where appropriate) as a minimum requirement. Data and Information are key assets and all users have a responsibility for the management and protection of these assets from deliberate, unintentional or unauthorised access, modification, destruction or disclosure.

* An Authorised Signatory is either the User’s team manager or a designated individual within the team/organisation with responsibility for staff/systems information maintenance (eg Technical Administration Officer). The Authorised Signatory must make sure the User is aware of the Corporate IT Security Policy and any other supporting policies and procedures.
