These questions use 7-point scales ranging from ‘Strongly Agree’, “Moderately Agree”, “Agree”, “Sometimes Agree/Sometimes Disagree”, “Disagree”, “Moderately Disagree” to ‘Strongly Disagree’. In each case there is opportunity for the student to indicate that a question does not pertain to their situation (N/A). Should you wish to exclude a particular standard question (#), please put a cross in the appropriate box (N/A) before copying the requisite number of forms.
