do hereby consent to our child trying out for the CG Dance Team and participating in the related Dance Team activities at CG.
We understand that participating in the Dance Team activities involves certain hazards arising from routines and activities. We do hereby agree not to hold the individual coach and/or School Corporation responsible should an accident or injury occur as a result of this participation.

It is not our desire to prohibit a student from trying out for the Dance Team because of financial obligations. If this is a concern, please see one of the coaches, and payment options can be discussed.
