CLS Program Manager

Responsible for case management and coordination of services to Kadima clients living in the community.

Responsibilities:

  • Hire, train, supervise and evaluate Community Living Services Staff.
  • Manage referrals for Community Living Services from private and public referral sources.
  • Determine appropriateness of services for new referrals; schedule and conduct intakes and assign clients to appropriate CLS worker.
  • Oversee and consult with CLS staff on development of treatment plan for private pay clients.
  • Train all CLS staff on use of treatment plans for publicly funded clients.
  • Monitor annual training for CLS staff.
  • Routinely monitor CLS charts for quality and compliance.
  • Routinely monitor tracking system to determine that CLS is being delivered according to treatment plans and Service Agreements.
  • Complete Corrective Action Plans as needed to resolve issues of quality and compliance.
  • Assist Finance Department to resolve billing and collection issues.
  • Participate with the treatment team and share relevant information about clients.
  • Communicate with client’s family members and providers and maintain appropriate documentation.
  • Attend supervision and trainings, staff meetings, quarterly review meetings, and medication reviews.
  • Document all contacts and information relating to the client.
  • Respond to client emergencies during and after office hours.
  • Follow Kadima mission statement and ethical code of conduct.
  • Abide by the National Association of Social Workers (NASW) Code of Ethics, Adult Protection Code, Mental Health Code, and the Commission on Accreditation of Rehabilitation Facilities (CARF) guidelines.
  • Perform any additional duties as assigned by supervisor.

Qualifications:

  • Minimum two years of college, or an associate degree in sociology, psychology, social work, or any related mental health field
  • Over the age of twenty-one
  • Experience working with adults with psychiatric disabilities preferred
  • Good written and oral communication skills
  • Must have a valid Michigan driver’s license, insurance and a good driving record
