Initiate a Referral
- In the open consultation click on the Referral heading and select Standard Outbound Referral:
- Add the Referral Source(the referrer) to the Referral Details box.
- Add the ReferralTarget,Carers Support West Lancashire.
- Use the Clinical Term Referral for assessment of needs of careror the Read code 8Hlw[lower case L].
- Check the Referral Date is correct and change it if necessary.
- Select an appropriate Purpose from the drop down list.
- Type the reason for the referralin the Referral Reason box.
- Depending on your processes you can create the letter now or selectCreate Letter Task For:in the Referral Letter section. If creating the referral letter now go to step 3 on page 3 after clicking OK.
Note: Creating a task enables the consultation to be saved and therefore added to the referral letter.
- If creating a task ensure the appropriate User/Team is selected and click OK.
- Save the consultation by clicking on the Save icon at the top left of the consultation screen, or press the F8 key on the keyboard.
The referral letter task will be assigned to the user or team that was selected and can be accessed by members or the team or user from the Pending Letter section of Workflow Manager.
Creating The Referral Letter
- Access Workflow Manager by the Quick Access Toolbar, the EMIS Button of the blue link to Referral tasks.
- Select Pending Letter in the Referral Management section of Workflow Manager and with the correct patient selected click Open Letter, or double click on the referral task.
- Select the WLCCG Carers Service referral letter template in the New Patient Letter list.
- Ensure only appropriate problems are ticked and click OK.
- Complete the referral letter adding any relevant information.
- Click File, Close and Complete.
- Click Yes to the message This will complete the task allowing no further actions.
Send The Referral by NHS.Net Email
Withthe generic NHS.Net referral email account open in Outlook having entered your password follow the steps below to send the referral.
- Go to the patient’s consultation screen and double click on the referral attachment (with the Paper Clip)next to the Document heading to view the referral letter.
- Click Send, Via Email.
- Type the Carers Support email address () in the To… box.
Note: If you have used the email address before you will see it in the popup list to select.
- Click on the Signature item on the Ribbon and select the signature you wish to add to the referral email.
- Click the Send button to send the referral to the Carers Service NHS.Net email address.
Note: The email will be listed in the Sent items folder of the account from which it was sent.
