5 minutes / Wing asked for input regarding verbiage for the Hold/Resume orders that the Pharmacy Informatics team is working on. The committee suggested that this be taken to the NIA meeting for nursing input / Wing shared the 5 new Hold/Resume options concerning medications.

This is not event based – meaning you cannot put resume or hold until after surgery or procedure.

Tom Mack for 8North / Under “turn /reposition” add “sitting side of bed” to dropdown menu. This whole concept has been referred to the PUPS committee. Discussion about this set of dropdown selections. / This is a PUPS bundle item so all suggestion must be taken to the PUPS committee for approval. Some areas of concern are the overlap between activities of daily living, which have specific documentation regarding location of activity (chair etc) and the specific documentation for the PUPS indicator which is triggered from the turn/reposition documentation only. NIAC Co Chairs are going to invite someone from the PUPS Committee to address this issue at a future NIAC meeting. / Invite member of PUPS committee to future NIAC meeting

5 minutes / Add more phone number lines in Care Contact section of Education record tab. Most family members have home, cell, and possibly a third number if they are visiting from out of town. / Field extended to allow more characters (both letters and numbers) to single line for care contact
Several suggestions were made to the committee.
·  It was suggested that there be box “See HED” in Admission History for contact information so that this is not being entered in two places.
·  It was also suggested moving it out of the Education Tab since it is required before you educate a patient
·  It was suggested that the name be changed to contacts instead of care contacts. / Change made November 13, 2012
The committee is going to take all of these suggestions under advisement and see what changes can be accommodated.

5 minutes / Discussion regarding removal of immunization link in Admission History for Adult patients only. / The form has been changed so that if the flu and/or Pneumovax immunization has already been administered the questions will be hidden.
This was causing an issue with immunizations that are automatically ordered according to protocol set in motion by admission history results being sent to the floor and not used – returned to the Pharmacy and the Pharmacy had to discard the meds.
It was also mentioned that if, the immunization portion of Patients History was not filled out it would flag the doctor when filling out the discharge orders. / Will leave the link until the committee receives more feedback on whether others are dependent on the link for immunization records.

Heather Campbell request to change labeling for Heparin fields that are causing confusion for those on her floor involved in pilot of documentation / Jay Morrison gave an update of the percentages of Heparin errors since the pilot began.
·  The base error rate was 55% and it is currently 21%
·  The volume base error rate was 41% and 24% and currently they are both down to 14% / Create a hover over Heparin (IV Med Drip) stating “dose” and a hover over Heparin stating “rate”
