If this form has been filled in by someone else on your behalf, you should read the answers carefully before signing the following Declaration.
Please remember that any omission or misstatement of a material fact in this form could affect the payment of benefits under any Policy or Certificate issued. A material fact is one likely to influence the assessment and acceptance of the Proposal by the Underwriters. If you are in any doubt as to whether a fact is material it should be disclosed.
I, the Life to be Insured and, if different, the Proposer, declare that the foregoing statements are true and complete answers to the best of my/our knowledge and belief. I/We undertake to inform the Underwriters of any changes to these statements which occur before the contract completes and I/We understand that failure to do so may affect the validity of the contract.
I, the Life to be Insured consent to Underwriters seeking medical information from any doctor who at any time has attended me concerning anything which affects my physical or mental health or seeking information from any insurance office to which an Application/Proposal has been made for insurance on my life and I authorize the giving of such information.
I, the Life to be Insured agree that this Proposal shall form the basis of the Insurance and will be incorporated in the Policy or Certificate of Insurance.
I have read my Statutory Rights under the Access to Medical Reports Act 1988 as explained on page 8 and I do not* wish to see a report before it is sent to Underwriters. I agree that a copy of this consent shall have the validity of the original.

NOTICE OF STATUTORY RIGHTS UNDER THE ACCESS TO MEDICAL REPORTS ACT 1988
Underwriters reserve the right to apply, with your consent, for a medical report from any doctor who has at any time attended you.
Please read this Notice of your rights in relation to such reports before signing the Declaration on page 7.
YOUR RIGHTS
* You can withhold your consent to the application for a medical report but without it your cover may be restricted.
* If you do give your consent you can indicate in the Declaration whether or not you wish to see any report before the doctor sends it to us.
* If you wish to see any report we must tell you if we apply for one and notify the doctor of your wishes.
* You will have 21 days to arrange with the doctor to see the report before he sends it to us.
* If you do see a report the doctor must obtain your consent before sending it to us.
* You have the right to ask the doctor, in writing, to amend any part of the report which you consider incorrect or misleading and you can ask him to attach a statement of your views on any part he refuses to amend.
EXEMPTIONS
The doctor does not have to let you see any part of a report that he considers would be likely to cause serious harm to the physical or mental health of yourself or others, or that would indicate his intentions towards you. He also does not have to let you see any part that would be likely to disclose information about, or the identity of, another person who has supplied information about you, unless that person has consented or the information relates to, or has been supplied by, a health professional caring for you. If the doctor does not let you see any part of the report he must notify you of that fact.
TIME LIMIT
Once the report has been supplied the doctor must keep a copy of it for six months and you are entitled to inspect it or receive a copy of if during that time.
PROCEDURES
If you indicate in the Declaration that you do not wish to see any report the doctor can send it to us immediately.
If at any time within the six months time limit you change your mind you should notify the doctor that you wish to see the report and arrange with him to do so or to supply you with a copy.
If you indicate in the Declaration that you do wish to see any report we will notify you if we apply for one and you will then have 21 days to arrange with the doctor to see the report before he sends it to us. This could, of course, delay the processing of the medical information.
The doctor is entitled to charge you a fee for any copy report supplied to you.
