Documentation of passing MTEL(Not your Massachusetts educator license). You can get a snapshot of MTEL results page from DESE website

Documentation of test registration.
Your MEPID Number
Completion of Supervising Practitioner Agreement Form (on ELI website) verifying that your principal or director is willing to assume the responsibility for being the administrative mentor during the practicum year 2017-18.
Phone interview. A member of the recruitment team will call you once all your application materials are submitted.
