How to register with the Practice

To register as a patient of the Practice, you may apply yourself or on behalf of any person incapable of making such an application themselves. Bring along your signed medical card, or in the case of a new baby the form obtained when registering the baby’s birth. If you have no medical card, our reception staff will give you a registration form to complete. You will need to bring proof of identity in order to register with the practice. Once registered, you may see any of our doctors, however you may prefer to see one particular doctor so that you can receive a more personal service.

Complaints and suggestions

If you have any queries, complaints or comments on the provision of services please let the Practice Manager know or speak to your Doctor. Suggestions are welcome. Complaints or concerns raised by patients will not affect the care received.

Rights and Responsibilities of the patient

Rights:

·  To be treated equally and with courtesy and respect.

·  To receive clear, understandable information about your treatment and care

·  To have privacy, confidentiality and dignity respected

Responsibilities:

·  Notify us if you cannot make an appointment – alternative arrangements can be made – benefiting others

·  We appreciate reasonable behaviour from our patients, their relatives and friends. Where a patient is violent or abusive to a GP, a member of staff or anyone on the Practice premises, they will be removed from the Practice list with immediate effect and further action will be taken as deemed necessary.

Confidentiality

Doctors and staff in and attached to the practice have access to your medical records to enable them to do their jobs. From time to time information may be shared with others involved in your care if it is necessary. Anyone with access to your record is properly trained in confidentiality issues and is governed by both a legal and contractual duty to keep your details private. We are a training practice and as such we are subject to inspection of our records, by other medical staff only, to ensure that we continue to meet the high standards required. We are also involved in research studies which require access to anonymous information from patient records. If you would like to opt out of this data collection scheme, please let a receptionist know and no data from your records will be collected or used in research.

You have a right to see your records if you wish. Please ask at reception if you would like further details. An appointment will be required and in some circumstances a fee may be payable.

Appointments

Appointments at either surgery may be made by telephoning 01228 529171 between 8.00 am and 6.30 pm Monday to Friday. We hold late evening surgeries Monday, Tuesday and Thursday until 8.00pm. You may consult any of the doctors in the Practice. URGENT cases will be offered an appointment within 24 hours, but it may not be possible to see your usual doctor.

Home visits

If you are too ill to come to the Surgery, please telephone 01228 529171 before 10.30am. Emergency calls only can be dealt with after this time. The Receptionist may ask the reason for the visit to allow the doctor to judge the urgency of calls.

Out of hours care

For situations requiring urgent treatment out of hours you should contact telephone 03000 247 247, however if you need emergency care, please dial 999 as usual.

Telephone advice

Should you wish to speak to a doctor or nurse for telephone advice, please telephone the Receptionist who will advise you of a suitable time to speak with your doctor or nurse or will arrange for you to be called back. Please advise the Receptionist if you think your problem is urgent or you think you may require a home visit.

Repeat prescriptions

If you are on regular treatment you may be given a repeat prescription request slip so that you can obtain further supplies before you need to see your Doctor again. It takes 48 hours (weekdays) to prepare a repeat prescription. If you wish, we can send your prescription to a local Chemist, but you should allow an extra day before collecting your medication from the Chemist.

You may also send your request by post.

Practice Nurses

Our Practice Nurses are available for immunisation/vaccinations, travel advice, ear syringing, removal of stitches, cervical smears, dressings, blood tests and general checks and advice. Other appointments for specific conditions are also provided by our Practice Nurses, including BLOOD PRESSURE, STOP SMOKING, DIABETIC CARE, ASTHMA AND WELL PERSON. These services are available throughout the day. Please ask the Receptionist for an appointment.

Practice Manager

The day to day management of the Practice is organised by our Practice Manager. Our aim is to provide expert and friendly personal health care for you and your family. Requests for information under the Freedom of Information Act should be directed to the Practice Manager.

Practice Receptionists

The Receptionists are here to help you with any queries you may have regarding the services the Practice provides. They arrange appointments for all the doctors and nurses, take requests for home visits and deal with other enquiries

Health Visitors

There are leaflets and posters around the Practice informing patients of Health Visitor clinics in the Carlisle area. Contact numbers for Health Visitors are written in baby’s health record book.

District Nurses

The District Nurses help in a person’s own home and have access to resources to help with this care. Contact them direct on 01228 608026 between 8.30am and 9.30am or leave a message on their answering machine.

General Practitioner training scheme

The Practice has been recognised as a training practice. Fully qualified doctors, who have already worked in hospital, join us to gain the special experience needed before entering a practice of their own. As part of the training programme you may be invited to have your consultation recorded by video. This will help towards the doctor’s training and will only be used for training and assessment purposes. Video recording will only take place with your full consent and intimate examination will not be recorded. Dr Beazley and Dr Docton are involved in GP training within the practice.

Medical and Nursing Students

Occasionally the Practice undertakes the teaching of medical and nursing students. The Receptionist will inform you when a student is in the Practice and we hope you will make them feel welcome.

Other Services

·  Contraception – Each GP provides a full range of family planning services.

·  Children’s immunisation – Clinic on Tuesdays at 1.30pm. If this is not convenient, other times may be arranged.

·  Travel immunisation – Available from the Practice Nurses. They will also be happy to advise on travel precautions.

·  Cervical smears – Arrange an appointment with a Practice Nurse

·  Maternity services – Each GP undertakes antenatal and post-natal care.

·  Minor operations – We undertake minor operations under local anaesthetic. Please discuss this with your GP.

·  Child health surveillance – The GP’s and the Health Visitors carry out child development checks.

·  Formal medical examinations – eg HGV, employment. Please ask the Receptionist for a special appointment.
