METHODOLOGY:
The following assessment report is the result of an in-depth assessment process that included all of the following:
  • Review of Positive Achievement Change Tool (PACT) screening instrument
  • Review of the offender’s legal history
  • A structured interview with the offender (SAMH-2)
  • Consultation with the current Juvenile Probation Officer
  • Conversation with offender’s parent/guardian
  • A 5 panel urinalysis

REASON FOR REFERRAL:
Although Javorio has no hits on his PACT screening client’s JPO reported that he is currently participating in Mental Health services at Multilingual Psychotherapy, and requested that client participate in a TASC assessment.
LEGAL HISTORY:
Facesheet indicated that the client is on probation for burglary unoccupied conveyance unarmed (F3), Larceny- grand theft (F3) and Larceny (F2). Please note that all of client’s other charges appear to be closed and/or Nolle Prosequi.
EDUCATIONAL/VOCATIONAL HISTORY:
Javorio is currently enrolled in the 11th grade at Forest Hills High School with a 3.7 grade point average. He shared that he likes school a lot because he is only taking AP classes. He shared that he has the luxury to graduate ahead of his class, but reported that he has decided to stay and complete the 12th grade so he can play football for the schoolteam. Javorio denied ever being expelled and/or retained from school. However, he disclosed that he was suspended approximately 10 times in middle school due to fighting, or being blamed for something he did not do, and for arguing with his teachers and peers. Javorio’s mother confirmed this to be true. Javorio shared that he has goals to get straight A’s so he can earn a scholarship for college and study engineering. He denied having any employment history but shared that he has vocational training from working alongside his father doing professional roofing and lawn mowing.
FAMILY HISTORY/ABUSE HISTORY:
Javorio’s mother shared that his JPO advised her that she has sent Javorio to live with his grandmother, uncle and cousin so that he is not at home in the community where he can be easily influenced by the wrong crowd. JPO confirmed this to be true and gave permission for the client to reside with his grandmother. Javorio shared that he gets along well with his family including his two brothers that live at home with his mother. He shared that when his family spends quality time together they would go to sports games, celebrate holidays, go out to eat, and attend family functions.
He shared that his grandmother and mother are the head of their households as they pay all of the bills. Javorio expressed that he has an 8pm curfew and is required to do chores around the house that include: “cleaning up my mess, taking out the garbage, cleaning the living room and kitchen, and washing my clothes.” Javorio shared that he doesn’t get into trouble at his grandmother’s house, but at home his mother disciplines him by raising her voice at him or talking to him. He identified his mother as the most important person in his life because “she is always there for me”.
Javorio denied every being kicked out of his house and or ever running away from home. He shared that his uncle would drink beers during football games and boxing matches but he denied any family history of substance abuse treatment and or counseling.Javorio’s mother disclosed that Javorio and one of his brothers are receiving services from Multilingual Psychotherapy Center. She shared that his brother was diagnosed with ADHD, ODD, and Manic depressive. She reported that Javorio was diagnosed with a mood disorder.
Javorio denied being the victim of any form of abuse, physical, sexual, and/or emotional. He denied ever intentionally hurting animals and/or setting fires. He denied ever being Baker Acted or having seen a psychiatrist or taking any psychotropic medication.
SOCIAL HISTORY:
In his free time, Javorio shared that he enjoys playing football and running track. He disclosed that his friends are good influences as they share the same vision to finish high school and go on to play college football. He denied having any involvement with gang related activities. Javorio described himself as athletic, relaxed, and smart.
SUBSTANCE ABUSE HISTORY:
Client was assessed for use of the following substances:
Tobacco / Denied use.
Alcohol / Denied use.
Cannabis / Experimented with marijuana at age 15, sharing a joint w/ 2 people. Client reported that he only took one puff and never tried it again. He stated that his last use was over 1 year ago. No cravings were reported, however he disclosed that he had experienced what he described as “body tingling” as withdrawal symptoms.
Crack/Cocaine / Denied use.
Ecstasy / Denied use.
Benzodiazepines / Denied use.
Inhalants / Denied use.
Opiates / Denied use.
Heroin / Denied use.
Hallucinogens / Denied use.
Amphetamines / Denied use.
Over The Counter / Denied use.
Flakka / Denied use.

URINALYSIS RESULTS:
Javorio was administered a U/A at the time of assessment and tested negative for all substances.
PHYSICAL HEALTH:
Javorio and his mother stated that he was last seen by a doctor in October 2017 due to breaking his hand during football practice. Javorio’s mother provided additional information and shared that he sustained a concussion when he was charged with car theft. “In May when he stole that car he got into an accident and the car flipped and he hit his head on the car roof and got a concussion.” It was reported that Javorio was treated at Jupiter Medical Center and referred to a neurologist. Lastly, his mother disclosed that Javorio “suffersfrom sports induced asthma and it is treated with an asthma pump and/or nebulizer that is prescribed by his pediatrician.”
MENTAL STATUS EXAM/PREVIOUS PLACEMENT:
Javorio was well-groomed and cooperative. He presented with appropriate mood and organized thoughts. His affect was congruent and he displayed no memory impairment. He was oriented to person, place, time, and situation at the time of the assessment.
Javorio admitted to a history of anger and/or fighting stating “I get mad if people insult me, touch me, don’t listen to me, or don’t respond to me when I am talking to them.”
Javorio is receiving services from Multilingual Psychotherapy Center to treat his mood disorder. His mother reported that he has been engaged in services since he was 3 years of age due to speech and behavioral problems. She further explained that he actively participates in individual therapy weekly at school and family therapy at home. He also sees his psychiatrist every 6 months to monitor his diagnosis as he is not currently on any medication. Javorio denied any past/present suicidal/homicidal, ideations/attempts and or plans.
SUMMARY AND RECOMMENDATIONS:
Javorio is 16 y/o Black male and was referred to TASC at the request of his JPO as there is no indications present on his PACT screening. Due to the information obtained at the time of assessment, Javorio is recommended to continue participating in services with Multilingual Psychotherapy Center where he is receiving weekly individual sessions and family therapy. It is further recommended that he continue to see his psychiatrist as scheduled.
