Existing Homes: / PCI currently has 5 homes in the Apple Valley area. At these homes we will promote community involvement, choice, and give our residents dignity and respect which they all deserve. We will serve 4 adults who have been diagnosed with various developmental disabilities and whom have severe behavioral problems. We will work closely with Inland Regional Center to serve the needs of our individuals who have been discharged from State Developmental Centers, or at the risk of State Developmental Center placement.
We will offer recreational activities to our residents including, but not limited to; board games, sporting games, and arts and crafts. Each resident will have their own bedroom and will be able to make choices on what activities or community invlovement they would like to participate in. The facility will make sure that each individuals health needs are met, and that we will work together with the residents to target and help eliminate behaviors which interfer with their daily lives.
Specialty Services: / Consumers will be evaluated by PCI's physician on a monthly and as needed basis to ensure their health needs are met. They will also be evaluated by a Psychiatrist on a monthly or as needed basis to ensure that their mental health needs are not only met, but also evaluated on an ongoing basis. They will also see a dentist and optomoitrist to make sure that their vision and dental health are evaluated.

Here at Apache we will ensure that we teach independence and self advocacy skills. We will help our individuals learn skills every day as indicated in their IPP's and will promote independence and inclusion in our community.
Staffing Pattern: / Apache consists of one Administrator which oversees the day to day functions of the facility to ensure that we are in compliance with all regulations and that the individuals and staff needs are met at the facility. The Administrator will be on grounds for a minimum of 20 hours per week while the residents are at the facility and will not oversee more than 10 beds total. The facility also has one House Manager who also needs 2 years experience working with individuals with developmental disabilites. There is a lead staff to assist the Administrator and House Manager and several Direct Support Professionals.
Inland Residential Services - Apache will offer 138 hours for the first (2) individuals in the home and an additional 108 hours for the 3rd and 4th resident thereafter.
The facility will require that each resident recieves 5.75 hours of 1:1 services from each of the following consultant areas;
Behavioral: Must be a psychologist, Licensed MFT, a Licensed Clinical Social Worker, or have a license which permits them to create and implement behavior plans. The behavior consultant will monitor the individuals, assess the progress notes, and hold In-Service trainings for staff to educate them on the residents behavior.
Recreational Therapist: Will be registered by the state of California. At least one year of working experience with individuals who have developmental disabilities. They will be expected to give group therapy sessions and produce a recreational plan. Give staff In-Service trainings to educate staff and will update the recreational needs of each resident.
Nurse Consultant: Will be a Licensed Nurse with the state of California. Have at least one year working with individuals with developmental disabilities, and meet all Title 17 Regulations. They will assess the healthcare needs and create healthcare plans for each individual who resides at Apache. They will constantly assess for possible psychotropic medication reduction.
A log will be kept by each consultant at the facility to document house spent and what they did for each resident every month.
Staffing Information:
Training Requirements: / The Administrator must be 21 years of age, have a certificate to be an Administrator in an Adult Residential Facilityand needs to have a minimum of 2 years experience working with individuals with developmental disabilities. Prior to hire, all of the positions require that they are DSP I, DSP II, CPR, First Aid, and CPI certified. All staff also are trained with our orientation form and receive 8 hours of in home training with the Administrator and or House Manager on grounds. DSP staff are also expected to receive 30 hours of CEU's the first year of hire and 20 CEU's every year thereafter. New Hire Orientation when they are first hired. They are also trained at the facility during an 8 hour on site training which covers topics such as the review of IPP's, DSP job description, and medication review. All staff also receive monthly training on Essential Learnings on various topics pertaining to individuals with developmental disabilities.

Special Features: Basketball court outside accessible to all individuals at any time. There is a park within walking distance from the facility to participate in community based activities. Patio furniture with shade to enjoy time outside of the facility while at home. There are flowers in the back and front yard area for residents to water and maintain. There is also a trail and open land in the back yard area for residents to play sports and walk around.
Safety Features: Alarm system added to all exits of the facility to sound when people are entering or leaving the house. Alarms with distinct sounds to alert when individuals are leaving there rooms and in what part of the house they are located at. A completely wide open floor plan with exits near each of the individuals rooms. Fire extinguisher in the front area and back area of the home and the house is equipped with a pull alarm system which is used in the case of a fire or an emergency.

Community Programs: / The park down the street from Apache has a community night where the residents can enjoy live bands from 70s, 80s and the 90s music.
Volunteer Programs: / Residents can participate at the retirement home in Apple Valley to help and assist individuals who are dependent on others. They can also join the salvation army in Victorville, CA to help stock shelves and assist in providing less fortunate individuals with clothing and other miscellaneous items.
Types of Transportation Services:
Inland Residential Services - Apache has a van readily available to the individuals in our home. We have a Non-Ambulatory van which will seat 5 passengers in the front and middle rows and 2 wheel chairs in back part of the vehicle. The town of Apple Valley also offers public transportation through a company called Victor Valley Transit which is located at, 11741 Santa Fe Avenue East, Hesperia, CA - (760) 948-3030
