When Speedway Children’s Charities awards a grant, we hope that the grant will have a positive impact on children in our community. This report is our primary opportunity to measure the achievements of the projects we support, and we use your results and feedback to determine where our funds have the best effect on children’s lives and what types of programs are best suited to our goals.
We are aware that your project may still be in progress, but we ask that you briefly answer the questions as they apply to your program up to this point. We are not only interested in what contributed to the success of your project, but also the challenges you face in reaching your ultimate goal.The evaluation form is to be completed and returned with any additional materials to Lisa Starnes, Speedway Children’s Charities, 5555 Concord Parkway S. 7th-Floor, Concord, NC 28027 byWednesday, August 1 at 5:00p.m.

1. Brief statement of project’s scope of work, goals and objectives for the past year, and the extent to which the goals and objectives were met.
2. Were there any unexpected outcomes of the project (either positive or negative)?
- Provide a success story describing how your foundation has benefited from Speedway Children’s Charities grant dollars. Please limit your summarization about the project we funded to no more than 350 words.
- Discuss the specific plans, if any, for continuing the work begun by this project.
- Briefly describe how your organization has been affected by both the United Way budget cuts, (if applicable), and the current state of the economy.
- Please provide copies of any PR your program received. Copies may include any materials that uses the SCC name and/or logo (examples may include print materials, press releases, website coverage, and/or signage).
- Please attach a list of itemized expenditures based on the project budget presented in the original grant application. Identify any variances.
- Did your group volunteer and/or partner with Speedway Children’s Charities and/or Charlotte Motor Speedway in the past year? If so, please explain.
