JOB TITLE: Community Health Nurse

BASIC FUNCTION: Under the direct supervision of the Community Health Program Nurse Manager, participate as a full functioning member of the Community Health Care Team in accordance with the Walpole Island First Nation philosophy, policies, and regulations. A) Major Responsibilities and Performance Requirements (only a few listed)

  • Assist in coordinating CHP services by participating in setting program priorities.
  • Conduct home visits within the community, discharge follow-up, health care, assessments, developing client care plans, providing health education.
  • Provide age-appropriate, preventative immunization services providers and agencies.
  • Conduct Public Health follow-ups on STI, Communicable disease, TB contact tracing and DOT, plus other PH referrals as required.
  • Maintain a current, accurate knowledge of health promotion, disease prevention.
  • Provide professional resource support services to other WIFN programs.
  • Perform other relevant duties as directed by immediate supervisor.

Qualifications:

B) Experience

  • Minimum of 5 years of experience in a relevant health care team setting
  • Minimum of 5 years of experience working with First Nations Community and Clients
  • Experience in conducting individual and family health assessments
  • Knowledge and experience with integrated quality management

C) Education

  • Minimum of RN registration with a BScN or BSN from a recognized university.
  • Current Canadian Red Cross CPR and First Aid Certification
  • Knowledge, skills and experience with computer programs such as MS Office etc.
  • Crisis and Suicide Prevention Training Certificate an asset.

D)Other

  • Must live and promote a healthy lifestyle.
  • Current College of Nurses Ontario Registration
  • Current RNAO Membership with LAP Insurance
  • Current Criminal reference and venerable sector check
  • Valid Ontario Drivers Licence and reliable vehicle
