Interschool Athletics Carnival

Your child has been selected to represent Freshwater Bay at the Interschool Athletics Carnival at Swanbourne Primary School. All students must wear their school sport uniform including running shoes (shoes with metal or plastic spikes are not permitted).

Year 5 and 6 children who were not selected as competitors will also be attending as our cheer squad. All parents are welcome to attend cheer for our Interschool Athletics team.

A program of events accompanies this note. The details of the carnival are as follows:

Interschool Athletics Carnival

Date:Wednesday, 14 September 2016

Time:Program commences at 11.00am and concludes at approximately 2.30pm

Venue:SwanbournePrimary School, Narla Road, Swanbourne

Transport:Return trip by bus leaving from Freshwater Bay Primary School at 10:25am returning by 3:00pm

Bring:School bag with lunch, water bottle, sunscreen and a hat. Please bring a raincoat if rain is forecast. Students must wear their school uniform.

Please complete, detach and return the permission slip below to your child’s teacher.

Permission and Consent Form:

I am aware that any costs incurred as a result of accident or illness are my responsibility and that school staff are not responsible for any loss or damage to my child’s personal property that may occur during the course of the excursion.

I agree to inform the organisers before the scheduled excursion of any change to my child’s health and fitness so the appropriate supervision may be arranged. I acknowledge that, should it be considered necessary, school staff will arrange to present my child for medical assessment.

I give permission and have read and agree to the above for my child to attend the Inter-School Athletic Carnival. Bus hire costs will be added to your child’s Term 4 Account.
