Children ages (X) to (Y) who live within the (name of league) area are eligible to enroll to play soccer. Registration will be required even if the child played previously.

Registration will be held at (registration sites). At least one parent or legal guardian must be present, with (number of) proofs of residence, and an original or state-certified copy of the child’s birth certificate.
