Applications are invited for a full time consultant psychiatrist posts within the NHS Greater Glasgow and Clyde Directorate of Forensic Mental Health and Learning Disabilities.

Thispost will be open to those with either Forensic Psychiatry or Learning Disability Psychiatry CCT’s and comprises of 10 Programmed Activities. The successful candidate would work predominantly with an outpatient caseload of about 30 patients across the Health Board area. He or she will lead a multidisciplinary team and be expected to assess patients for suitability for specialist intervention. Joint working and liaison with community learning disability teams and services across the patch is an essential component to this post and much of the work will involve shared care in this respect.

This is a tremendous opportunity for dynamic and enthusiastic individual to join our service and take a full part in the ongoing development of the largest forensic service in Scotland. The service includes established forensic community mental health and learning disability teams covering the Health Board area; a 53 bed low secure service at Leverndale Hospital, Glasgow; and medium secure care at Rowanbank Clinic, a 74 bed purpose built unit at Stobhill Hospital, Glasgow which covers the West of Scotland region and provides the national medium secure learning disability service.

On call responsibilities will be shared with other consultant forensic psychiatrist colleagues on an approximately 1:13 rota. The Successful candidate will be encouraged to maintain or develop special interests as part of their professional and career development. This could include prison sessions, a specific subspecialty outpatient clinic or an academic role between the service and the university department.This post will be supported by a medical secretary.Core and specialist trainees are also attached to our forensic service.There are active education and audit programmes within the service.

Applicants must have full registration with the General Medical Council, a licence to practice and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of Higher Specialist Training leading to a CCT in Forensic Psychiatry or Learning Disability Psychiatry or eligibility for specialist registration (CESR); or be within six months of confirmed entry at the date of interview. Non-UK applicants must demonstrate equivalent training.

CONSULTANT FORENSIC PSYCHIATRIST

COMMUNITY LEARNING DISABILITIES

The Post

Applications are invited for this full time Consultant Forensic Psychiatrist Community Learning Disabilities post. This is a tremendous opportunity for a dynamic and enthusiastic individual to take a full part in the ongoing development of the largest forensic mental health service in Scotland.

The successful candidate will provide consultant leadership to a multidisciplinary Forensic Community Learning DisabilityTeam (FCLDT) and will be expected to assess patients and provide liaison to local learning disability services. The post is open to those with either forensic or learning disability CCT’s.

The post holder will have administration support and willbe expected to work closely with colleagues to provide a full range of services to outpatients and ensure their safe and effective management in the community.He/she will be encouraged to maintain or develop special interests, as part of their professional and career development.

The FCLDT consists of:

  • 1 WTE consultant psychiatrist
  • 1 WTE consultant forensic clinical psychologist
  • 2 WTE forensic/learning disability community psychiatric nurses

The FCLDT is one of 3 teams within the NHSGGC forensic community service (see below). Its remit includes:

  • Joint working with community learning disability services across NHSGGC;
  • Provision of a liaison service to the community learning disability teams;
  • More direct management of a relatively small group of higher risk patients (including restricted patients) in the community (probably no more than 30 patients at any one time).

The specific aims of the team are to:

  1. Strengthen existing community learning disability teams by providing specialist forensic support as necessary.
  2. Provide forensic assessments and reports including court reports for forensic patients and potential offenders.
  1. Undertake risk assessment work, including assisting community learning disability teams with complex risk assessment and management plans.
  1. Provide second opinions to members of the community learning disability team where the case continues to be held by that team.
  1. Have lead responsibility for high risk cases and the psychiatric management of cases where forensic need is paramount.
  1. Have lead responsibility where there is a need for ongoing risk management/monitoring of offending behaviour until mutually agreed handover to the relevant community learning disability team.
  1. Promote the provision of a co-ordinated range of services (including access to health, criminal justice services, social care, housing, education and employment and benefits advice) to meet the individual needs of learning disabled offenders and serve the public interest. The public interest covers both the protection of the public and the most effective use of resources consistent with high standards of public safety.
  1. Provide specialist forensic treatments e.g. for sexual offending behaviour.
  1. Provide training to community learning disability team staff e.g. in risk assessment.
  1. Undertake preventative work with potential offenders.
  1. Facilitate access to specialist inpatient services as appropriate.

Present Service Provision

The NHS Greater Glasgow and Clyde (NHSGGC) Health Board area has a total population of 1.2 million. Acute services within NHSGGC are delivered through three Directorate and three Sector structures. These are the North Sector, South Sector, Clyde Sector, Woman and Children Directorate, Diagnostic Directorate and Regional Services Directorate.

The Forensic Mental Health and Learning Disabilities sub-directorate (FMHLD) is one of five sub-directorates of the Regional Services Directorate which provides services to Glasgow, the West of Scotland, and in some cases all of NHS Scotland. FMHLD also maintains close links with NHSGGC Mental Health Services in terms of clinical governance, medical appraisal and continuing professional development.

FMHLD employs approximately 420 staff. The Clinical Director is Dr Martin Culshaw and the General Manager is Mr James Meade. The management team operates from a range of bases including Rowanbank Clinic, Leverndale Hospital, Clutha House and the Douglas Inch Centre (based at the West Ambulatory Care Hospital, Yorkhill).

The FMHLD’s clinical staff include forensic psychiatrists, clinical psychologists, nurses, occupational therapists and allied health professionals who specialise in working with mentally disordered offenders. The clinicians are fully supported by an administration team providing secretarial and administrative services to ensure the smooth running of the service. For inpatients, care is provided through a progressive, patient focussed approach within a safe and secure environment. Treatment interventions are underpinned by risk assessment and management to maximise the rehabilitation potential of the individual patient. The focus is on maximising education and employment opportunities with a view to supervised or independent living in the community in the future.

The core components ofFMHLD are:

  1. Medium secure service: Rowanbank Clinic is a 74 bedded, purpose built, medium secure forensic unit next to Stobhill Hospital. It is Scotland’s largest medium secure unit, providing a medium secure service to the West of Scotland region. Within the Clinic, there is a 10 bed male admissions unit and 38 male rehabilitation beds across 3 wards. There are 10 female beds across 2 wards including 4 beds for female learning disability patients. There is a separate 8 bedded male learning disability ward. The learning disability beds form the national medium secure learning disability service for Scotland, funded via a National Risk Share Scheme.
  1. Low Secure service: there are53 forensic low secure inpatient beds provided at Leverndale Hospital- 30 male mental illness beds, providing assessment and treatment includingforensicrehabilitation; 9 close supervision male learning disability beds; 9 pre-discharge (open) male intensive rehabilitation beds: and 5 female beds.
  1. Community service: this has developed over a number of years into a robust service for the whole of the NHSGGC area, currently split into 3 teams- one each for the Glasgow and Clyde areas and the separate Forensic Community Learning Disability Team.
  1. Forensic liaison services, including adiversion service operatingin4 Sheriff Courts across NHSGGC; and psychiatric input to 4 prisons (HMP Barlinnie, HMP Low Moss, HMP Greenock and HMP Cornton Vale).

Medical Staffing

The successful applicant will join 12 other forensic consultants in NHSGGC and take part in a combined on call rota on a pro rata basis.

Duties of the Post

The successful candidate will work predominantly with an outpatient caseload of about 30 patients across the NHSGGC area. Depending on the configuration of the medical workforce at any one time, the post-holder will also undertake a ‘group task’ clinical session e.g. one prison session per week or an equivalent clinical session in an additional area of forensic psychiatry to broaden their job plan and allow the development of specific expertise.

The successful candidate will lead a multidisciplinary team and be expected to assess and manage patients deemed appropriate for input from the service. Joint working and liaison with community learning disability teams and services across the patch is an essential component to this post and much of the work will involve shared care in this respect.

The successful candidate will participate in the on call rota and cover for colleagues during absence in line with arrangements set out in the national Terms and Conditions. The forensic on call rota covers the whole of NHSGGC and, on the appointment of this post, full time consultants will be on call 1 in 13. Court reports, Mental Health Act/Adults with Incapacity Act and forensic liaison assessments will be undertaken as required. Duties also include continuing clinical responsibility for the patients in your charge allowing for all proper delegation to, and training of, staff.

Specific responsibilities of the post will include the following:

  1. Consultant leadership to the Greater Glasgow and Clyde Forensic Community Learning Disability Team (FCLDT).
  1. Fostering a multidisciplinary service which values and seeks out the contribution of respective partner agencies and professional groupings.
  1. Responsible Medical Officer for restricted patients and other patients subject to compulsory measures looked after by the FCLDT.
  1. Responsibility for assessment and ongoing treatment of patients referred to the service.
  1. Participate in an on-call rota (currently 1 in 13).
  1. Liaison with forensic and learning disability colleagues.
  1. Liaison with consultant psychiatrists, general practitioners, other medical colleagues and area learning disability teams with regards to assessment and management of complex forensic learning disability cases.
  1. Provision of consultant psychiatrist cover to HMP Greenock prison for urgent referrals when colleague is on leave.
  1. Contribution to strategic development and ongoing development of the service and redesign as required to meet the needs of the patient group.
  1. Implementation of the shared values implicit in local and national policies as they apply to learning disabled offenders, including the involvement of service users and carers in planning, delivering and monitoring standards of service.
  1. Teaching and supervision to individuals from varied backgrounds and in particular trainee medical staff.
  1. Establishing and maintaining close professional links by attending and taking part in local and national case conferences, journal clubs and other teaching.
  1. Participation in the medical advisory structure through the divisions of psychiatry and other committees.
  1. Home visits, court reports, CPA, MAPPA, Mental Health Act/Adults with Incapacity Act and liaison assessments will be undertaken as required.

The exact nature of commitments will be a matter of agreement between the post-holder, Clinical Director and General Manager.

Teaching/Administration

It is expected that the successful candidate will take part in continuing professional development and participation in audit or research is encouraged. The post holder will be actively involved in the teaching of undergraduate medical students. In addition the post holder will be expected to participate in the postgraduate teaching of medical staff and staff of other disciplines.

The post holder will undertake the administrative duties associated with the care of patients and the proper function of the clinical team. He or she will be expected to participate in the broader aspect of administration and management within the service.

This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director. NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings. The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.

JOB PLAN AND TIMETABLE

A job plan will be agreed between the post holder and Clinical Director.

This 10 programmed activity (p.a.) (=40 hour) post consists of:

1 p.a. of supporting professional activity

9 p.a. of direct clinical care

Direct Clinical Care time will include emergency and on call duties and any ‘group tasks’ relevantto the service and the individual by negotiation with the ClinicalDirector.

The job plan could also be arranged to suit a part time individual in discussion with the ClinicalDirector.

Section 22 Training

Completion of day 1 and day 2 Section 22 training is required prior to taking up post. The Medical Staffing Department will require sight of your certificates to this effect.

QUALITIES REQUIRED

Essential

  • Full registration with the GMC, a licence to practise and be on the Specialist Register for Forensic Psychiatry and/or Learning Disabilities within 6 months of gaining the award of CCT or equivalent.
  • MRCPsych or equivalent.
  • An ability to interact professionally with colleagues, patients and carers.
  • A commitment to and working knowledge of multi-disciplinary working.
  • Leadership skills.
  • Knowledge of current issues in forensic psychiatry and learning disabilities and an understanding of the therapeutic use of security.
  • Experience of working in a community mental health setting
  • Section 22 approval.

  • Publications in the medical literature and/or presentations at Royal College or similar events.
  • Experience in providing teaching at undergraduate or postgraduate level.
