JOB DESCRIPTION: CARE MANAGER – NURSING HOME
PURPOSE AND OVERVIEW
To take overall responsibility for the provision and monitoring of high quality care services, including taking a lead role for Audit within the home, and to support the General Manager in ensuring robust clinical governance.
Responsible to: General Manager (Nursing Home)
Responsible for:Supervision of all staff involved in delivery of Care, either directly or through Registered Nurses.
Job Purpose
As a Registered Nurse the Care manager will comply with standards of professional practice and conduct at all times, and will maintain a valid registration with the NMC in order to practice.
The Care Manager will:
- Be part of the Home’s Senior Management Team with a lead responsibility for management, support and direction of care services.
- Support the General Manager to develop the Nursing and Care Team, ensuring that the home has the necessary clinical skills and underpinning knowledge to manage the needs of residents.
- Assess, plan, implement and evaluate care of residents using evidence based practice.
- Ensure a high standard of care service is provided for residents and will take charge of the Home in the absence of the Manager.
- Be responsible for the day to day care of residents, delegating roles to nursing and care staff as appropriate
- Provide clinical supervision to the Registered Nurses, and through supervision ensure that the Registered Nurse team obtain and sustain the clinical competencies necessary to meet the needs of residents
- Actively participate in personal and professional development opportunities, attending mandatory training as identified
Leadership and Management
- Manage and direct staff within areas of responsibility, ensuring they operate efficiently and flexibly, and are focussed on meeting the need of the customers.
- Deputise for the Home’s Manager during their absence, taking overall responsibility for the management of the Home.
- Take an active role in Risk Management and Health & Safety in line with company policy, ensuring any accidents, incidents or undue occurrences reported or witnessed are addressed.
- Oversee staff rotas in the home.
- Operate the company appraisal and personal development system encouraging all staff to gain relevant new skills and experience.
- To work closely with, and support the General Manager in ensuring the effective and efficient management of the Home at all times.
- To work with the General Manager in ensuring the Home complies with all aspects of registration by the Care Quality Commission (CQC), taking lead responsibility in matters regarding care and clinical governance, including representing the Home at any inspections.
- Lead the nursing and care staff at the Home to ensure the delivery of a high quality, continuously improving care service.
- To promote and operate the quality management system.
Care Quality / Customer Focus
- Carry out monthly audits, including monthly audits of care plans to ensure completion and accuracy.
- To utilise the agreed performance/audit systems, including regular auditing of procedures and processes in all matters relating to clinical and care practice.
- Provide support to Registered Nurses to ensure a consistent approach to care planning based on personalised care and evidence based practice
- Anticipate resident and needs whenever possible to enhance the quality of service offered by the teams.
- Be responsible for the day to day nursing and care of residents, delegating roles to care staff as appropriate.
- Be responsible for the day to day planning, provision and recording of resident care in compliance with all legislative and regulatory requirements.
- Liaise with residents, families and carers, the General Manager, staff and other relevant parties to ensure effective communication, and to develop and review resident’s care plans.
- To lead in the management of medicines in the Home, overseeing the ordering process, undertaking monthly audits and maintaining annual competency assessments for those staff responsible for the administration of medication
Financial Management
- To work with the General Manager in preparing an annual budget for the Home.
- To monitor and control relevant budgets to ensure the Home achieves business and financial targets.
General
- Undertake other duties from time to time as directed by the General Manager in relation to the good management of the Home.
- To ensure compliance with all policies and procedures relevant to the service.
- To represent the Home in all matters relating to care and clinical matters to outside bodies.
