Boarding Agreement and Waiver of Release ~ Please read carefully~
GENERAL TERMS Cancellation policy includes 7 days notice prior to dogs stay and 15 day notice during holidays. I agree that during peak busy times, Christmas, Thanksgiving and Spring break, Music Festival, that my partial-payment towards boarding is non-refundable without 15 day cancellation notice.
I acknowledge that dogs can incur scrapes, scratches, bites, sprains, strains, broken bones, cuts and other injuries or even death. Dogs can be exposed to communicable viruses and parasites such as kennel cough, lice, fleas, ticks and other diseases and parasites even with medication and vaccination. I acknowledge my dog is not currently ill, or has not been ill or contagious within the last 10 days,as this may affect my dogs stay at this time.PLEASE INITIAL HERE_____
VACCINATIONS/HEALTH:
All animals must be current on vaccinations for Rabies, DHLPP and Bordetella (kennel cough). The owner must provide Wagitude Pet the veterinarian records on paper for these vaccines prior to boarding. All animals accepted for boarding must be free of fleas and ticks; owner will be charged a $40.00 fee for any animal that is dropped off with parasites. Guardian hereby represents that this pet has not been exposed to Rabies, Bordetella, Distemper, or Parvo within the last 30 days. I acknowledge my dog ______is current on all vaccinations, including Bordetella and any anti-parasite medication. I have provided up- to- date vet and vaccination information.Iunderstand that if my dog is a senior, or has anxiety, that it may become stressed. I acknowledge my dog is fit and healthy enough for play, exercise, and boarding.I give Wagitude Pet permission to take necessary medical, or other, action should an emergency arise during my pets stay, and guardian shall be notified at once for instructions.Owner will cover this expense, including $20 per hour emergency transportation fee. Owner agrees to release Wagitude Loved Dog Hotel and hold harmless its owner from any responsibility and or liability for injuries and conditions, illness, escape or loss of life to your pet that may occur during boarding, transport, play, or exercise. I disclose that my dog has not been aggressive toward other dogs.
This waiver restricts you or any family member from seeking any legal action whatsoever against Wagitude.

Wagitude Pet will exercise reasonable care for the safety of your pet and to keep the boarding premises safe and secure. WagitudeLoved Dog Hotel liability for any boarded pet will not exceed $28.00. The pet guardian is solely responsible for any property damage or bodily injury inflicted by the boarded dog. Wagitude cannot guarantee against accidents. We will not be liable for loss or damages to your pet’s belongings.

Wagitude reserves the right to immediately change your dog’s boarding if we believe it is necessary to protect the health and well-being of your dog, or other dogs.
The house brand of dog food is Annamaet Dry Food. Changing a dog’s food can cause upset stomach, owners are welcomed & encouraged to bring their dog’s food from home. There is a daily fee of $3 per day for feeding our brand. Owners are welcome to bring their dog’s own bedding or toys if desired, however we cannot guarantee that they will be returned in the same condition. This agreement covers the current relationship between Wagitude Loved Dog Hotel and yourself. Each time you bring your dog to Wagitude, you affirm the terms of this Agreement, and the truthfulness and accuracy of all statements you make in this Agreement.
