Job Description: Basic Center Program (BCP) Street Outreach Worker/Case Manager

Definition

The Street Outreach Worker/Case Manager is responsible for conducting street outreach and providing information and referrals, client intakes, case management, and direct services to youth and families referred to the Childrens Cabinet. This position reports to the BCP Coordinator.

Duties

·  Conducts outreach in specified sites to identify youth and families in need of services and as it relates to BCP guidelines.

·  Works with BCP Coordinator to develop street outreach calendar; assists with scheduling and implementation of Street Outreach programs in school settings and community-at-large (downtown, Truckee River, skateboard parks, etc.).

·  Responds to Safe Place calls.

·  Works with school counselors, social workers and youth probation officers to identify and serve youth at-risk of running away and homelessness.

·  Case manage BCP identified youth staying at Children’s Cabinet’s Center for Aspiring Youth (CAY), youth are identified during street outreach and through Safe Place Program.

·  Conducts follow-up activities with youth clients.

·  Enter, update, and monitor information in computerized client database(s) for the BCP.

·  Achieve an understanding of agency mission, strategic focus areas and overall goals and objectives as well as contributing to the success of those areas.

·  Coordinate the referral and intake processes for street outreach, including information and referral for self-referred clients and contacts with the Washoe County School District, Washoe County Juvenile Services, and other community referral sources.

·  Disseminate information regarding Street Outreach and the Safe Place Program, including community presentations and program materials, and perform other outreach functions as needed.

·  Submit monthly reports regarding program activities to the BCP Coordinator.

·  Set annual goals and objectives related to specific job function.

·  Perform related duties as assigned.

·  Attend weekly meetings with BCP Coordinator to discuss street outreach progress and activities.

·  Standby help for the Center for Aspiring Youth.

Minimum Requirements Needed

Knowledge of

·  Community agencies and resources, as well as an understanding of the problems facing children, youth and families.

·  Family and youth relationship issues

·  Appropriate interactions with youth and families

·  Case Management skills

·  Leadership skills including sound decisionmaking, crisis management techniques, staff motivation and be a selfmotivated team player.

·  Computer applications and the desire to learn new programs.

·  Crisis management

Ability to

·  Self motivate

·  Learn new ideas and skills by reading and with hands-on training

·  Present information to various diverse groups

·  Express ideas clearly and concisely both orally and in writing

·  Work with adults and youth of diverse backgrounds and beliefs

·  Problem solve in stressful situations

·  Keep appointments punctually, be organized and demonstrate follow through

·  Implement case management strategies with identified families

Special Requirements

·  Bachelor's degree or equivalent experience which includes program planning, case management, and crisis management experience. Equivalent experience may be substituted for the degree.

·  Bilingual (English/Spanish) preferred.

·  Must be able to work nights and weekends.

·  Must complete required online training.

·  Must have reliable transportation; may be required to transport youth.

·  Must possess a valid Nevada Driver’s License and proof of insurance if utilizing a personal vehicle for company business.

·  Must be able to pass background check.
