2015 PQRS Measures Applicable to RDNs
Important
Starting in 2015, one PQRS measure previously used by RDNs can now only be reported through EHR reporting.
Diabetes: Hemoglobin A1c Poor Control (PQRS #1) (NQS Domain: Effective Clinical Care)
- Percentage of patients 18-75 years of age with diabetes who had hemoglobin A1c > 9.0% during the measurement period
Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up (PQRS #128) (NQS Domain: Community/Population Health)
- Qualifies as a cross-cutting measure
- Percentage of patients aged 18 years and older with a documented BMI during the current encounter or during the past six months AND when the BMI is outside of normal parameters, a follow-up plan is documented during the encounter or during the previous six months of the encounter.
- Normal Parameters: Age 65 years and older BMI≥ 23 and < 30; Age 18-64 years BMI≥ 18.5 and < 25
Documentation of Current Medications in the Medical Record (PQRS #130) (NQS Domain: Patient Safety)
- Qualifies as a cross-cutting measure
- Percentage of visits for patients 18 years and older for which the eligible professional attests to documenting a list of current medications using all immediate resources available on the date of the encounter. This listmustinclude ALL known prescriptions, over-the-counters, herbals, and vitamin/mineral/dietary (nutritional) supplements ANDmustcontain the medications' name, dosage, frequency and route of administration
Elder Maltreatment Screen and Follow-Up Plan (PQRS #181) (NQS Domain: Patient Safety)
- Percentage of patients aged 65 years and older with a documented elder maltreatment screen using an Elder Maltreatment Screening Tool on the date of encounter AND a documented follow-up plan on the date of positive screen
Registry-Based Reporting Only
Diabetes Mellitus: Diabetic Foot and Ankle Care, Peripheral Neuropathy - Neurological Evaluation (PQRS #126) (NQS Domain: Effective Clinical Care)
- Percentage of patients aged 18 years and older with a diagnosis of diabetes mellitus who had a neurological examination of their lower extremities within 12 months
Diabetes Mellitus: Diabetic Foot and Ankle Care, Ulcer Prevention - Evaluation of Footwear (PQRS #127) (NQS Domain: Effective Clinical Care)
- Percentage of patients aged 18 years and older with a diagnosis of diabetes mellitus who were evaluated for proper footwear and sizing
Preventive Care and Screening: Unhealthy Alcohol Use - Screening (PQRS #173) (NQS Domain: Community/Population Health)
- Percentage of patients aged 18 years and older who were screened for unhealthy alcohol use at least once within 24 months using a systematic screening method
EHR Reporting Only
Diabetes: Low Density Lipoprotein Cholesterol (LDL-C) Control (<100 mg/dL) (PQRS #2) (NQS Domain: Effective Clinical Care)
- Percentage of patients 18-75 years of age with diabetes whose LDL-C was adequately controlled (less than 100 mg/dL) during the measurement period
RDNs who report PQRS quality measures data can request to receive National Provider Identifier (NPI)-level Physician Quality Reporting Feedback Reports. The reports include information on reporting rates, clinical performance, and incentives earned by participating individual professionals, with summary information on reporting success and incentives earned at the practice level. The feedback reports can be accessed through theQuality Net web portalin the fall of the year following the reporting period (e.g., 2015 feedback reports will be available in the fall of 2016).
