Title page (upload as a separate file):

Article type / Indicate the article type at the top: Original Research, Analytical Report, Review, Meeting Report, Commentary, or Letter to the Editor
Title: Subtitle / List full title in bold. For clarity, separate the subtitle from the title with a colon.
Authors / List all authors in final order. Include academic and professional degrees.
Affiliations / Provide institutional affiliation (do not include job title), city, state, and country.
Contact details / Provide corresponding author name, mailing address, and email.
Acknowledg-ments / Provide any acknowledgments and/or disclaimers.
Conference presentation / Provide the name, date, and location of the conference or workshop at which the paper was presented, if appropriate.
Conflict of interest / (1) List any potential conflicts with respect to the research, authorship, and/or publication of this article; or (2) indicate “no conflicts of interest”
Funding / List any funding sources or indicate “no funding sources.”

Manuscript:

Title: Subtitle / List full title in bold at the top of the manuscript file. Information identifying authors or their affiliations should be removed from this file.
Abstract* / Abstract length: 150-250 words. *Original research articles should have a structured abstract with a Background, Methods, Results, and Conclusion section. Abstracts for Analytical Reports, Reviews, Meeting Reports, and Commentaries do not need to be structured. No abstractneeded for Letters
Keywords / List 5-6 keywords for indexing and online searchability.
Text / The article body should be organized into Background, Methods, Results, Discussion, and Conclusions sections. Other descriptive headings and subheadings may be used, if appropriate. Do not number the headings.
Clinical/research studies involving humans/animals must include institutional review board/ethics committee approval statement in the Methods. If a clinical/research study did not involve animal or human subjects, include this statement: “This article does not contain any studies with human or animal subjects performed by any of the authors.”
Tables / Include a citation in the article text for each table. All tables must be supplied as editable Word- or Excel-based tables (not scanned). Number each table in order of appearance, provide a short title, and insert it in the text after the paragraph where it is first cited.
Figure captions / Include a citation in the article text for each figure. Number each figure caption in order of appearanceand insert it in the text after the paragraph where it is first cited. Upload the corresponding image file separately.
Figures will be in color online and converted to grayscale for print,so figure captionsshould avoid terms indicating color.
If using an image that has been previously published, written permission must be obtained from the original publishers and submitted with the manuscript. (Permission is not needed if the image has been modified or adapted.)
References / Number in order of citation appearance and format according toAMA Manual of Style guidelines.Each reference must be cited in text. References “in preparation” or “unpublished” should be placed withinthe text in parentheses.

Figure file(s):

Each image should be at least 15 cm (6 inches) wide, with text proportional to image size. Submit original image files (eg, Word or Excel charts) when possible. Scanned images should be a minimum of 300 dpi resolution and saved in JPG, EPS, TIF, or PDF format. Name each file with the corresponding figure number and upload each file separately.
