1. Office of the Deputy Director, Animal Resources Development & Parishad Officers at South 24 Parganas (Alipore), North 24 Parganas (Barasat), Howrah, Purba Medinipur at Tamluk, Dakshin Dinajpur and Jalpaiguri.
2. Central Medical Stores, Sub-Depots at Nadia, Bankura, Uttar Dinajpur, Murshidabad, Cooch Behar, Siliguri, Bardhaman, Hooghly (Chinsurah), Birbhum, Purulia, Paschim Medinipur & Malda.
3. Joint Director, Animal Resources Development, Presidency, Bardhaman and Jalpaiguri Division.
4. Joint Director, Animal Resources Development, State Livestock Farm, Kalyani, Nadia/ Composite State Animal Husbandry Farm, Salboni, Paschim Medinipur.
5. Additional Director, Animal Resources Development, Institute of Animal Health & Veterinary Biologicals, 37, Belgachia Road, Kolkata-37 / Haringhata Farm, P.O. Mohanpur, Dist. Nadia.

Uniform Rates must be quoted applicable to the entire above mentioned Unit. No separate rate is to be quoted for each of the above Units which will not be considered for acceptance.

(1)Catalogue (Non-Refundable) together with Terms & Conditions and Tender Form for submission of Tender are obtainable from the office of the undersigned on Non-Refundable Cash Payment of R.200/- (Rupees Two Hundred) only for Drugs & Laboratory Chemicals/ Surgical Equipments, & R.2/- (Rupees Two) only towards the cost of each Tender Form per item etc. on any working day except Public Holidays & Saturdays between 12 Noon & 4 pm (barring lunch hours between 1-30 pm to 2 pm) up to 09.02.2011.

(2)Those who intend the download the Tender Documents from Website are required to deposit R.200/- (Rupees Two Hundred) only plus R.2/- (Rupees Two) only for each Tender Form [Non-Refundable] towards cost of the same in Demand Draft on any Nationalized Bank Payable at Service Branch, Kolkatain favour of the “Director of Animal Husbandry & Veterinary Services, West Bengal” along with the offer.

(3)Request for Catalogue along with Fees by Post will not be entertained.Each Tender of the above two Groups (i) & (ii) must be supported by:

(a)Earnest Money Deposit either in the form of “Deposit-at-Call”/ Bankers’ Cheque/ Pay Order amounting to R.10,000/-(Rupees Ten Thousand) only on any Nationalized Bank in favour of the ‘Director of Animal Husbandry & Veterinary Services, West Bengal, A/c. : ______(Name of the Tendering Firms/s) in original.

Valid Exemption Certificate issued from the competent authority for claiming exemption of depositing Earnest Money such as valid S.S.I. Certificate issued by Government of West Bengal, D.G.S. & D. Rate/ N.S.I.C. Certificate, etc.

OR

ORIGINAL copy of T.R. Challan No.7 for R.10, 000/-(Rupees Ten Thousand) only shows that the said amount has been deposited in favour of the “Director of Animal Husbandry & Veterinary Services, West Bengal”under the Head “8443-00-103-001-07”.

For Medicine only:

(b) Turnover of the Manufacturer (outside the State of West Bengal) direct importer (holding an Import License). State based Public Sector Unit, State based Non-Public Sector Unit & others should be at least R.40,00,000/- (Rupees Forty Lakh)only and that of S.S.I. It should be at least R.10,00,000/- (Rupees Ten Lakh) only. Documentary evidence to that effect is to be submitted along with tender. Participant below the said stricture are not eligible & their Tender will summarily be rejected.

(c) Valid Drug License or current Validity Certificate in case of Drug License is under renewal (i.e., up to 31.12.2010).

(d) Valid current “Non Conviction Certificate” (i.e., up to 31.12.2010).

(e)Manufacturing License with list of items for which Rates have been quoted/ offered by coloring the item of Drugs quoted in the tender relating to the manufacturer.

(f)IMPORTANT: FirmslocatedoutsideofthisStatewillhavetofurnishdocumentaryevidence towards enlistment/placement of supply order of tendered item(s)bytheirownState Govt. as credential.

For Surgical items (some are Common to Medicine also):

(g) VAT Registration Certificate.

(h) Professional Tax Clearance Certificate along with Challan valid up to 31.12.2010.

(i) Documentary evidence of Manufacturing Surgical Equipments/Instruments issued by the competent authority (for Surgical Items only).

(j) Authorized Dealership Certificate (for Surgical Items only).

(k) PAN Card.

(l) Original Demand Draft/Pay Order [summation ofR.200/- (Rupees Two Hundred) & R.2/- (Rupees Two) only for each tender form] on any Nationalized Bank Payable at Service Branch, Kolkata in favor of the “Director of Animal Husbandry & Veterinary Services, West Bengal” towards fees for downloading of tender documents or Photocopy of D.C.R. towards procurement of Tender Documents by cash payment.

(m) A copy of tender terms & conditions duly signed in each & every page is to be submitted along with tender as a token of acceptance.

Documents at (a) to (k) must be duly attested by Gazetted /Group-A Officers, if photo-copies are given indicating the full name of the attesting person. Un-attested photocopy will not be considered as valid document.

VAT Registration Certificate if not furnished with the Tender, must be furnished at the time of receiving payment (vide G.O. No. 3246-FT dated 29.9.1997) only once against delivery.

NO TENDER WILL BE ACCEPTED IF THE SAME IS NOT SUPPORTED WITH ALL THE ABOVE DOCUMENTS MENTIONED AT SL. NO. (a) TO (k).

Rates should be quoted NET in decimal coinage (both in words & figures) inclusive of delivery, packing, Insurance, loading & unloading charges, etc. for supply of articles of all of the pre mentioned units. THE OFFERED RATES SHOULD BE VALID FOR AT LEAST 2 (TWO) YEARS.

Rates of any item, if not offered for whole of the decentralized units, will not be considered for acceptance.

It is obligatory on the part of Bidder to enclose the duly attested copy of D.C.R. in support of purchase of tender paper.

TENDERS OF ITEMS IN THE GROUP OF C., H., I., J., OF CATALOGUE OF DRUGS & LABORATORY CHEMICALS & SURGICAL REQUISITES MAY BE SUBMITTED BY DEALERS/ TRADERS/ SUB-DEALERS, ETC. BESIDES MANUFACTURERER WILL ALSO BE CONSIDERED FOR ACCEPTANCE.

Ratesof VAT & Excise Duty are to be indicated extra, if charged. Tincture & Spirituous preparations are to be quoted “Duty Free”. The rates quoted shall hold good for 24 months from the date of approval (up to 31/3/2013). Rates of any period as will be specified by the undersigned or Tender Committee at the time of finalization of Tender and to be extended, if situation arises. Rates of items would be written distinctly and clearly and any over writing, if not authenticated by hand and seal of the bidder, will summarily be rejected. Supply is to be completed within 30 days from the date of issue of order except for those orders placed during the month of March, 2013 or supplies in respect of which are to be completed within 10.3.2013 and so on.

Sealed tender super scribing “Tender for supply of Drugs & Laboratory Chemicals, etc.” or “Tender for Surgical Equipments, etc.” for the year 2011-2013 as the case may be by addressing the undersigned along with the Name & Address of the bidder should reach the office of the undersigned on or before 10th February, 2011 within 3 pm. The tender box and as well as sealed tender will be opened in presence of bidders or their authorized representatives as per notification to be notified in the notice board of this Directorate starting from 11 am on 03.02.2011.

For further information, the bidders are requested to please contact the undersigned.

The undersigned reserves the right to reject/or all tender/s at any stage without assigning any reason thereof and will not be bound to accept lowest tender.

TERMS & CONDITIONS for ‘Supply of Drugs, Laboratory Chemicals &Surgical Equipments etc.

for the year 2011-2013’.

1. Sealed Tenders are invited ONLY FROM THE MANUFACTURERS for Supply of (i) Drugs & Laboratory Chemicals etc. (For few items mentioned hereunder to which Authorized distributor/dealer/trader may participate) & (ii) FROM MANUFACTURERS OR AUTHORISED DEALERS / BONAFIDE SUPPLIERS / TRADERSfor Surgical Equipments, etc. Rate/s is/are to be submitted only in the prescribed ORIGINAL TENDER FORM for the year 2009-2011 duly signed and affixed with the stamp of the Firm at each page forming part of the Tender. Bidders may quote any number of items BUT SEPARATE TENDER FORM in duplicate must be used in each item mentioned in the catalogue. Only one rate should be quoted on the prescribed ORIGINAL Tender Form thus purchased for only one item / specification. IF MORE THAN ONE RATE FOR A SINGLE ITEM IS QUOTED IN A SINGLE TENDER FORM, THE RATES FOR THAT ITEM IRRESPECTIVE OF MENTIONING OR NON-MENTIONING OF UNIT PACKING WILL BE TREATED AS CANCELLED. Bids shall be addressed to The Director of Animal Husbandry & Veterinary Services, West Bengal and should be submitted in sealed cover super scribing “General Tender for Drugs & Laboratory Chemicals / Surgical Equipments, etc. for the year 2011-2013” as the case may be along with Name & Address of the Firm/Bidder.

ITEMSINTHEGROUP-C,H,IJOFCATALOGUEOFDRUGS, LABORATORYCHEMICALS,ETC. SUBMITTED BY DEALERS/TRADERS, SUB-DEALERS, ETC BESIDES MANUFACTURER WILL ALSO BE CONSIDERED FOR ACCEPTANCE.

a)Each Tender, unless the Bidder is exempted under the existing orders of the West Bengal or Central Government will have to be submitted with either Bankers’ Cheque /Pay Order/Deposit-at-Call worth R.10,000/- (Rupees Ten Thousand) only on any Nationalized Bank in favor of “Director of Animal Husbandry & Veterinary Services, West Bengal, A/c.: ______(Name of the Firm)” . Showing the Earnest Money of R.10,000/- (Rupees Ten Thousand) only has been deposited in favour of the Director of Animal Husbandry & Veterinary Services, West Bengal, under the Head, “8443-00-103-001-07” in Treasury Challan No 7. The onus of proving that a Bidder is exempted from deposit Earnest Money will be on the Bidder and must be proved by submission of valid documents to that effect such as S.S.I. Certificate of Government of West Bengal/ D.G.S. & D. Certificate, etc. The original Bankers’ Cheque/ Pay Order/ Deposit-at-Call Receipt or T.R. Challan No.7 must be furnished along with the Tender for (i) Drugs & Laboratory Chemicals, etc. and (ii) Surgical Equipments (Separately), etc.

In no circumstances, will Cash, G.P. Notes, Savings Bank Pass Book, Fixed /Term /Special Term Deposit Receipt,/ Token of Reserve Bank of India or A/c. Payee Cheque, etc. save & except those mentioned above be received with Tender as Earnest Money.

b)The Earnest Money Deposit shall not carry interest. This Directorate is not liable for deposition of excess of Earnest Money.

c)Earnest Money deposited before the date of issue of this Tender Notification will not be accepted. Earnest Money Deposit of previous tender will not be adjusted/considered for this tender.

d)The onus of proving that a Bidder is exempted from the depositing Earnest Money will be on the Bidder & must be proved by submission of valid documents to that effect, e.g., Small Scale Industries should furnish S.S.I. Registration Certificate along with Annual Production Return of the preceding year. Co-operative Societies of this State should furnish Registration Certificate from the State Registrar of Co-operative Societies.

e)Registration of S.S.I. Units after submission of Tender shall not entitle the Bidder to exemption from depositing Earnest Money.

f)Those who intend to download the tender documents from Website are requested to deposit R.200/- (Rupees Two Hundred) only for Tender Documents & Rs.2.00 (Rupees Two) for each tender form non-refundable in DEMAND DRAFT on Service Branch, Kolkata & on any Nationalized Bank in favor of Director of Animal Husbandry & Veterinary Services, West Bengal with the offer.

g)Earnest Money will be released after finalization of tender & in the following cases:-

1. Tender Documents which are not valid.
2. In other cases, it will be released in due course.

(a)ORIGINAL or duly attested (the attesting officer either Group-A or Gazetted) photo-copies of PAN, Trade License, VAT Registration Certificate, Professional Tax Paid Challan Clearance Certificate, Drug License, Manufacturing License, No Conviction Certificate (all valid up to 31.12.2010) etc along with Brochure of quoted item/s are to be furnished along with tender.

(b)FOR MEDICINE ONLY:

(d)Bidders, if they do not furnish VAT Registration Certificate along with offer, must have to furnish the same at the time of receiving payment against bill otherwise payment, will not be made.

(e)Duly certified copy of English version of pre-requisites as in (i) are to be furnished.

(f)In no case, Affidavit of Trade License/VAT Registration Certificate will be accepted.

(g)Original Trade License/ VAT Registration Certificate, Professional Tax Paid Challan, Exemption Certificate, Manufacturing License, Drug License & No Conviction Certificate or any other document are required to be furnished when called for.

(h)Authorized Dealers/ Distributors should furnish all documentary evidence to that effect along with offer.

(i)Attested photo-copy of D.C.R. (Duplicate Carbon Receipt) is to be enclosed as a proof of purchase of Tender Document. Unattested photo-copies will not be treated as Valid Document.

(j)Tenders will have to be submitted in duplicate in prescribed form duly signed & affixed with seal of the firm by indicating full communicating address with PIN Code, Telephone/ FAX/E-mail/ Website number, in a sealed cover, super scribing the Tender Notification No. & Date and to be addressed to The Director of Animal Husbandry & Veterinary Services, West Bengal. A Tender Box will be preserved in the Purchase Cell of this Directorate. No Tender will be accepted across the table & no such receipt will be issued thereon. Envelopes must be closed & sealed properly with any sealing material.

(k)This tender schedule is to be submitted duly signed in all pages with tender as a token of acceptance.

(l)Legal documents (made on Non-Judicial Stamp Paper worth R.10/-) regarding appointment of Agent/ Stockiest/Distributor, if manufacturer or bidder wants to supply through Agent/Stockiest/ Distributor.

(m)Up-to-date Valid Drug License issued under the Drugs & Chemicals Act, 1940 along with full list of endorsement of drugs by indicating Page No., Item No., in the endorsement list of the Drug License by coloring those, valid and up-to-date manufacturing license with the list of approved items Drugs License will not be required for quoting Rates only for P. Alum, Linseed Oil, Commercial Bleaching Powder, or validity certificate in case the Drug License is under renewal.

(n)“Non-Conviction Certificate” from the Drugs Control Authority (up-dated) if the quoted items comes under the Drugs Control Act, 1940 (i.e., valid up to 31/12/2010).

(o)Documentary evidence of manufacturing Surgical Instruments/ Equipment issued by competent authority.

(p)In case the Bidder is exempted from VAT or if he enjoys the benefit of deferment he must submit documentary evidence to that effect.

(q)Documentary evidence of manufacturing instruments/equipments issued by the competent authority.

(r)Attested photo-copy of Laboratory Analysis Report (Test Report) for ISI Marked items for the last batch manufactured must be enclosed.

(s)It is compulsory for the bidder for Medicines to enclose their printed /leaflet booklet (neither photocopy nor cyclostyled) namely Pharmaceutical Index or Therapeutic Index containing product list details of each items for which rates are quoted.

(t)G.M.P. / I.S.O. Certificate, if there be any, (attested photo-copy of the same) must be produced at the time of submission of tender.

(u)“No Conviction Certificate’’ from the Drug control Authority (updated) if the quoted item comes under the Drug control Act. 1940 (i.e. Valid up to 31/12/2010).

(v)Documentary evidence of manufacturing Surgical Instruments/ Equipments issued by the competent authority.

(w)In case the bidder is exempted from VAT or if he enjoys benefit of deferment he must submit documentary evidence to that effect.

(x)Attested photocopy of Laboratory analysis report (Test report) for ISI marked items for the last batch manufactured must be enclosed.
