VENDOR SPACE: Payment and reservation is for 2 days. Vendors will be uniformly set up throughout school building (Classrooms, Gym, Cafeteria, and Halls) – therefore, some spaces could be 10 x 10, or 5 x 20. For more information, contact the High School Main Office at (716) 933-6005. Upon completion of this application, confirmation and further instructions/information will be provided to vendor.

I understand that if I have misrepresented my merchandise or violate any rules and regulations, Portville Art & Antique committee reserves all rights of cancellation and disqualification without refund. I release the committee from all responsibility for installing, protecting, selling, storing and removing my exhibit and merchandise. Portville Central School is not responsible for any items, property loss or damage.
