PRACTICE GUIDANCE - PATIENT ACCESS TO CLINICAL SYSTEM

Before you begin to use Patient Access we would appreciate if you could read the following guidance. Please keep the document for your own reference.

Terms and Conditions

Application

  • To apply for on-line access to the Practice’s clinical system, patients must complete an application form and return this form to the Practice Manager
  • Applications are “one per patient”. Acceptance of one member of a family does not imply acceptance of other / further family members
  • Where access is granted passwords will only be released direct to the patient or their representative and not to a third party. The practice will not provide access detail directly without the patients’ consent
  • Requests for re-issue of access log-in details will be via post reissue in all cases to the registered address

Appointments

On-line appointments booked are to be cancelled by the patient as soon as it is determined that it is no longer required.

Nurses’ Appointments

Due to the nature of nurses’ appointments we are unable to offer them on line. If you are unsure as to whether it is appropriate for you to see a nurse or a GP please contact us by telephone.

Prescription Requests

  • Patient Access will give you a facility to order your repeat medication on line
  • You will only be able to view and request repeat medication
  • Any other medication cannot be ordered using this facility and must be requested in person, by fax or via your usual chemist
  • We will still require 2 working days to process the request and longer if it is sent to a chemist
  • Please only order the medication you require and the medication must be due.
  • You must state where you wish to collect the prescription ie Chester Rd Surgery, Seymour Grove HC or a particular chemist. WE ARE UNABLE TO PROCESS ANY ON- LINE REQUEST WITHOUT THIS INFORMATION.

View medical record summary & immunisation record

You will be able to view your medical summary detailing your medical problems and immunisation record

Inappropriate use

We monitor the use of the on- line service and we are sure that you will find it useful. However, if we find any abuse of the service, we will revoke your access. We would consider inappropriate use such as sending appropriate or abusive messages, booking appointments and not using them (more than twice a year).

Your responsibility

The Practice will take every measure to ensure that your Patient Access application is secure. It is your responsibility to ensure that your Patient Access account remains this way. You are able to reset or terminate your access account at any time by contacting the Practice Manager. You may wish to do this if you think someone else knows your log in details.
