Fee Policy and Financial Agreement

**If a child/ren attends part-time only, any changes to the above schedule must be approved by the Director.

RATES AND PAYMENT POLICIES

Effective January 1, 2008, allpayments must be made electronically with a checking or savings account. A Direct Debit Authorization form must be completed before a child’s first day at LilyPad. Special arrangements should be discussed with the Director if unable to make electronic payments.

We have agreed to pay the following rate/s for child care services:

Payments will be debited weekly, biweekly, or monthly. I agree to the following method of payment:

*Weekly and biweekly payments are withdrawn on Mondays for the upcoming week/s of care.

*Monthly payments are withdrawn on the 1st Monday after the 1st of the month for the upcoming month of care.

FEES

Enrollment Fee

A non-refundable enrollment fee of $50.00 must be paid to reserve a spot for your child/ren. Until that enrollment fee is paid, your spot may be filled.

Late Pick-Up

LilyPad closes at 6:00 PM, Monday through Friday. Parents will be charged $5.00 for every 15 minutes a child is presentafter 6:00PM. Teachers will record late fees for processing. If there is a late pick-up fee, parents will receive a written notice from the Director of the billing amount in the week following the late pick-up. All late fees must be paid by the end of the month.

Returned Checks/ Insufficient Funds

All returned checks or direct debit payments rejected due to insufficient funds will be subject to a $25.00 penalty.All fees should be paid by the end of the month.

VACATION AND HOLIDAYS

Vacation Days

Children attending LilyPad full-time receive 7 vacation days per calendar year. In order to receive credit for vacation days, please notify the Director at least one week in advance. Vacation days cannot be used on paid holidays. **Children attending LilyPad part-time are not eligible to receive vacation days.

Paid Holidays

The following are paid holidays for which LilyPad will be closed:

New Year’s Day / Thanksgiving Day
Memorial Day / Friday after Thanksgiving
4th of July / Christmas Eve
Labor Day / Christmas Day

Extended Absences

In the event that a child needs to take a leave of absence, such as summer break, and wishes to return to the program after a period of time, a fee of $50.00 per month will be due for every month the child is absent. More than two consecutive months of non-payment and no contact could result in termination of enrollment. Please discuss absence arrangements with the Director.

Termination of Enrollment

In order to keep the center at full enrollment levels, please provide the Director with a two week notice when your child must withdraw from the program so the spot may be filled. If a two week notice is not received, you may be charged for those two weeks.

I agree to pay all child care fees in a timely manner and to adhere to all policies of this financial agreement. I understand that violation of these financial terms could result in fees or suspension/ termination of enrollment without prior notice.
