Your student is applying the Accelerated 4/1 MPH in Environmental Health at the University of Minnesota School of Public Health. Information about the program is available at

To apply to the School of Public Health, all information is submitted through SOPHAS, the centralized application service for Schools and Programs of Public Health.

We are asking students applying to the program to submit this degree progress form in an effort to ensure that it is both possible and realistic to successfully incorporate the additional 12 graduate level SPH classes into their senior year. Students must earn at least a B- or better in SPH classes for them to count toward the MPH. This form will substitute for one of the traditional three letters of recommendation.

The student should identify you as a recommender within SOPHAS. You will be sent a link to SOPHAS asking you to accept or decline the role of recommender. Once you have accepted you will be prompted to create an account, a process that will take only a few minutes. Then, you will see the recommendation panel (a screenshot is included below).

Please complete the Relationship, Likert Criteria, Overall Recommendation portions, and upload the form below in place of a letter. You must complete something on the Likert Criteria - if you mark “not observed” that will not be held against an applicant, as we are most interested in the practical considerations of completing the program and understand that you may not know the student well. However, if you feel you can capably speak to any of the criteria, that assessment is welcome.
