POSITION OBJECTIVES:

  • To ensure the provision of quality, consistent, responsive and meaningful support services to clients within the framework of creating independence including the Active Service Model and Wellness and Reablement Framework.
  • To provide leadership and support to Intake Workers, Assessment Administration and Assessment Officers (as required) through supervision, training and performance management.
  • To assist in the achievement of high standards of efficiency, client service and accountability to Aged, Disability and Emergency Response Unit and funding bodies.
  • To undertake home based holistic ‘living at home’ assessments of clients and their carers requiring access to Council’s Aged and Disability Services, ensuring that the services offered are responsive to client goals and care needs.
  • To foster a coordinated approach to the provision of services and support to clients by establishing excellent professional relationships with other agencies.

  1. KEY RESPONSIBILITY AREAS:

Management and Leadership:

  • Provide leadership, support and supervision to Intake Officers, Administration – Assessment Service Officer including identifying and addressing performance issues and training opportunities.
  • Effectively manage operational issuesto ensure continuous effective service delivery and a performance based, innovative, responsive and accountable person centred work culture
  • Manage and coordinate the allocation of work within the team, including planning, performance monitoring and reporting, staff recruitment, performance management and supervision in line with council guidelines.
  • Liaise with Unit Manager, Assessment Services to ensure consistent program management including compliance with all reporting requirements.
  • Ensure that the comprehensive system of client records and statistical data for accountability to Council and funding bodies is maintained by the accurate and timely input of data
  • Participate in relevant meetings both within Council and externally ensuring that knowledge of Council and external services and programs is up to date.
  • Maintain links and work collaboratively with relevant service provides within the region.
  • To provide leadership and support within change management processes.

Service Delivery:

  • To Undertake home based, ‘living at home’ holistic assessments of complex clients requesting access to Council’s Aged and Disability Services taking into account their expressed goals and care needs.
  • To provide short term case management services to eligible clients as required(depending upon aged care reform requirements)
  • To provide secondary consultation as required to Assessment Officers regarding complex clients, case management practice and support planning.
  • Ensure every client has an agreed written care plan that is goal directed and time limited with a review date and if appropriate, has a referral action plan and /or information provided on health promotion and social activities
  • Liaise as required with external service providers or agencies to assist in the ongoing development of service delivery utilising active service and wellness and reablement frameworks.
  • Comply with all governmentfunding, legislative and corporate quality, OH&S and service standard requirements.

Continuous Improvement

  • Implement and maintain Continuous Improvement System standards and procedures.
  • Manage, Support and assist with training of relevant staff members to ensure service delivery standards are reviewed and improved as identified.
  • Contribute to the development of plans and policies for the Aged, Disability and Emergency Response Unit.

Environmental Sustainability

  • Incorporate Council’s environmental sustainability objectives and targets into projects and programs.
  • Promote and participate in a culture of environmental sustainability.

Occupational Health & Safety

  • Implement and maintain health and safety standards and procedures according to legislation and consistent with MoreSafe.
  • Demonstrate effective leadership on OHS matters
  • Ensure compliance with OHS reporting and monitoring procedures
  • Promote sound industrial practices and maintain a safe work environment adhering to Occupational Health and Safety requirements.

JOB CHARACTERISTICS RELEVANT TO THE POSITION

The following Job Characteristics should be read in the context of the definitions describing the characteristics required of a Band 7 employee as outlined in Part B of the Enterprise Agreement(Victorian Local Authorities Award 2001, Appendix A, Part A - Employee Band 7, Clause 7)

  1. ACCOUNTABILITY AND EXTENT OF AUTHORITY:

This position has authority to provide, develop and interpret policy and to provide information and advice to members within the Assessment Services Unit.

Freedom to act is governed by Funding and Service Agreements, Council and Unit Policies and Procedures and relevant Acts and professional standards. Freedom to act is generally prescribed by the Unit Manager Assessment Services.

Accountable for:

  • Supporting the development and implementation of the My Aged Care framework into the organisation
  • Assisting in the development and interpretation of policy and procedures
  • Standard of provision of assessment and intake service
  • Maintenance of appropriately skilled and supervised Intake Officers, Administrative Officer within unit and Assessment Officers as required.
  • Ensuring a high standard of client/customer service and satisfaction within Assessment Services Unit
  • Ensuring good OHS Outcomes by monitoring relevant OHS standards

JUDGEMENT AND DECISION MAKING:

Judgement is exercised withinthe framework of operational and budgetary guidelines and the provisions of relevant Acts, Regulations, Codes and Council Policies and Procedures.

The incumbent’s decisions will have a significant effect on the performance of Council’s Aged and Disability Services team and the public perception of Council as a whole.

Decisions are governed by Organisational goals and policies, statute and subordinate legislations. Significant decisions will be made in conjunction with the Unit Manager, Assessment Services.

Problem solving and policy development occurs in the context of identification and analysis of a broad range of options, with the identification and development of policy and procedural options of the consideration and endorsement of management.

This position is also responsible for handling escalated issues and cases.

  1. SPECIALIST KNOWLEDGE AND SKILLS:
  • Detailed knowledge and experience in the Aged and Disability field and in the provision of Living At Home Assessments within a person centred approach.
  • Understanding of the legislative, statutory, policy and program requirements relevant to the provision of Commonwealth Home Support Services and in particular the responsibilities of My Aged Care and Assessment Services.
  • Detailed knowledge of Active Service Model/Wellness and Reablement Frameworks and Person Centred Care Practice.
  • Detailed knowledge of Case Management practice and principles would be desirable.
