Use this form to give notice to take a period of Shared Parental Leave, giving at least 8 weeks’ notice of the start date of the leave. You must have previously completed the Notice of entitlement form and had your eligibility for Shared Parental Leave confirmed.

[1] Length of service requirement: 26 weeks’ continuous service with the College by the end of the 15th week before either the expected week of childbirth or adoption.

[2] Length of service requirement: one years’ service with the College at the qualifying week (15th week before the expected week of childbirth or adoption)

[3] Please note once the request for the SPL period has been approved, this will normally only be changed if one of the exceptions outlined in the policy apply.

[4] Please note: you can make a maximum of three requests during the SPL period.
