DundeeFertility Associates GCRM Satellite

Dundee Fertility Associates (DFA) provide satellite services atMaryfield Medical Centre, Dundee, for patients who wish to attend GCRM, Glasgow for fertility treatment. This means that several visits to Glasgow are avoided, asinitial consultation and completion of paperwork (registration for treatment) can be carried out in Dundee. During treatment, ultrasound scans and blood tests can also be carried out in Dundee, with trips to Glasgow only required for egg collection and embryo transfer, or intrauterine insemination.

Consultation and Registration Requirements

To ensure that registration for treatment at GCRM can be completed at your consultation appointment, please bring the following:

  1. Photographic ID (both partners)

A colour photocopy of your passport details page is required. Please bringyour passports (or photo driver’s licence) to allow the consultant to confirm that it is a true copy.

  1. From your GP:
  2. Proof of Rubella immunity (female partner)
  3. Negative Chlamydia test result (female partner)
  4. Smear test result (within normal recall)
  5. Diagnostic semen analysis report
  1. Completed Forms (enclosed):
  2. GCRM Registration Form
  3. Female social questionnaire(GCRM Welfare of Child)
  4. Partner social questionnaire(GCRM Welfare of Child)
  5. HFEA Consent to Disclosure forms (both partners)

Any questions relating to the completion of these forms can be discussed at your consultation.

  1. Treatment Summary:

If treatment by IVF or ICSI has been undertaken elsewhere a detailed summary is preferredto help plan the next treatment. (A summary is not required if your last treatment was at Ninewells Assisted Conception Unit).

If this is yourfirst treatment cycle,Hepatitis B, Hepatitis Cand HIV blood results must be no more than 3 months old when you commence your stimulation drugs. For subsequent cycles they must be within 24 months of the start of your stimulation drugs.

If you are using donor sperm then only the female being treated is required to complete these tests.

At the registration visit you may also need afemale fertility assessment, which involves an ovarian reserve blood test (AMH) and a vaginalultrasound scan of the womb and ovaries. Fees charged are indicated below. You should bring a debit/credit card and be prepared to pay for these at your appointment

What happens after registration?

After your registration visit, your DFA consultant will send the required information to GCRM,who will contact you by telephoneThis telephoneconsultation will cover the details of your treatment protocol,as well as taking payment for your treatment and arranging for medication to be delivered by Alcura. Alcura will contact you directly to take payment for the drugs and arrange a suitable time to deliver them to your home.

Once your drugshave arrived, you should make an appointment with the DFAspecialist fertility nurseat Maryfield Medical Centre,who will teach you how to give yourself the injections, complete treatment consent forms, and clarify any other issues.Copies of the completed consent forms will be sent to GCRM. These must be at GCRM in Glasgow before you can start treatment.

When you are ready to start treatment, you will then be advised to contact GCRM(0141 891 8749), usually nearthe start of a menstrual period. Exact dates for starting medication as well as monitoring scans(and blood tests) will be advised by GCRM and you should thenbook appointments directly with DFA at Maryfield Medical Centre (01382 462292). Please call between 4-5:30pm to arange appointments.

Monitoring results from DFA will besentto GCRM, who willthen contact you directly to advise of thedate for your next scan and blood test, and keepDFA up to date with your ongoing treatment and management.Once treatment has progressed to the stage where egg retrieval is due, you will attend GCRM for the procedure. The same will apply foryourembryo transfer.

After the embryo transfer, GCRMwill advise you of the date you should attendDFA for your pregnancy blood test.
