6) BRIEF RESUME OF INTENDED WORK

6.1) Need for the study

Nidra is considered as one among the Trayopstambhas1 in Ayurvedic Classics and is needed for physical and mental maintenance of the body. In the present day, various Sleep-related disorders are increasing day by day.Working tendency during the night-time has increased due to busy lifestyle. Insomnia is affecting nearly one third of adults in western developed countries and the incidence is increasing day by day in developing countries also.2If Insomnia is left untreated it may reduce the mental capacity and efficacy and may drag the individual towards several psycho - somatic disorders.

Modern management for Insomnia is mainly tranquilizers. Initially tranquilizers give considerable relief but continuous and long term use may cause various side effects including drug dependency. Need has always been felt to develop certain Ayurvedic treatment modalities for the management of Nidranasha (Insomnia) which is safe, effective, readily available, cost effective and without any side effects.

Among Moordhini Taila Shirobasti stands high and superior in attributing maximum therapeutic effect especially in Prajagara 3(Insomnia). On the other hand, Padabhyanga which has been side lined from the main stream of Ayurvedic Treatment needs to be given considerable importance as it is told to be beneficial in inducing Nidra 4. Hence an Effort is made to evaluate the Efficacy of Shiro Basti with and without Pada Abhyanga along with the administration of Placebo.

6.2) Review of literature

·  Charaka mentions Nidra as one of the Trayopastambhas.1

·  Vagbhata mentions Nidranasha as a symptom of Vata Vruddhi.5

·  Charaka and Vagbhata mention the causes for Nidranasha.6 7

·  Vagbhata mentions lakshanas of Nidranasha.

·  Charaka mentions Moordhni taila as a treatment for Nidranasha.8

·  Susrutha mentions Pada Abhyanga as treatment for Nidranasha.9

·  Tungadrumadi Taila is mentioned in Tailaprakarana in Sahasrayogam.10

·  Detailed description of Insomnia is mentioned in Harrison principle of Internal Medicine11 and Comprehensive Text Book of Psychiatry.12

6.3) Objectives of study

·  To Evaluate the Efficacy of Shiro Basti with Pada Abhyanga in Nidranasha.

·  To Evaluate the Efficacy of Shiro Basti without Pada Abhyanga in Nidranasha.

·  To Compare the Therapeutic Effect of two groups.

7) MATERIALS AND METHODS

7.1) Source of data:

A minimum of 40 patients of Nidranasha will be selected randomly by Purposive Sampling Technique from OPD and IPD of SKAMC Hospital & Research centre, Bangalore.

7.2) Method of Collection of data

·  This is a comparative clinical study with pre test and post test design where in a minimum of 40 diagnosed Nidranasha patients of either sex will be randomly assigned into two groups, each comprising of 20 patients.

·  A case proforma specially designed for the study will be prepared with all points of history taking, signs and symptoms as mentioned in the text.

·  The parameters of signs and symptoms will be scored as per the proforma.

·  Following statistical methods will be employed for the data collected –Descriptive Statistics, Contingency table analysis and Repeated measure ANOVA using SPSS for Windows Software.

Inclusion criteria

·  Patients Presenting with the Lakshanas of Nidranasha.13

·  Patients of either sex aged 20 years and above.

·  Diagnosed Patients of Insomnia.

Exclusion Criteria

·  Below 16 yrs of age

·  Patients unfit for Snehana.14

·  Patients unfit for Abhangya.15

·  Other systemic diseases which interferes with the course of treatment.

STUDY DESIGN

This is a Comparative clinical study with Pre-test & Post-test design will be conducted

on 40 Patients, with minimum 20 patients in each group having signs and symptoms of Nidranasha.

DURATION OF THE STUDY

·  It will be for 7 consecutive days .

·  Pre test -1st day , Post test – 7th day , Follow up – 14th and 21st day.

INTERVENTION

40 patients of Nidranasha (Insomnia) who fulfils the Inclusion criteria will be selected and randomnly assigned into following 2 groups each comprising of 20 patients.

Group –A

·  The patients of this group will be subjected to Shiro Basti with Tungadrumadi taila till the

·  appearance of samyak lakshanas - for 7 days.

·  Pada Abhyanga will be done with Tungadrumadi taila for 15 min - for 7 days.

·  Placebo in form of capsule filled with starch powder will be given 2 capsules at bed time - for

·  7 days.

·  Follow up will be done on 14th and 21st day.

Group – B

·  The patients of this group will be subjected to Shiro Basti with Tungadrumadi taila till the

·  appearance of samyak lakshanas – for 7 days.

·  Placebo in the form of capsule filled with starch powder will be given 2 capsule at bed time

·  For - 7 days.

·  Follow up will be done on 14th and 21st day.

Assessment criteria:

Assessment will be done based on following -

Sleep diary assessment –

·  Time into bed

·  Time of lights out

·  Time to fall asleep

·  Number of awakenings

·  Time out of bed

·  Naps – day time

·  Rate how you felt today

·  Irritability

·  Total time of sleep

·  Sleep quality.

·  Apakti.

Parameters will be assessed using different grading before and after the treatment.
