TEAM ROSTER:Maximum 19 players including goalies. Please list all participating players and officials on the Roster Form.

ACCEPTANCE:Final Acceptance of Teams applying for Port Moody Amateur Hockey Association Tournaments cannot be considered until Port Moody Amateur Hockey Association has received a complete application, roster and payment.

Tournament Permission #:
We will require a Tournament Permission Number. The Permission Number must be submitted no later than 48 hours before your first game (if your team is not a Pacific Coast Member then a letter from your association will be required).
Deadline: Registration Forms and Team Roster will be accepted up to one week before the first day of the tournament or until the tournament is full. Cheques must be made payable to PMAHA and received within 7 days of your registration submission.
By signing this Registration Form, the Team Manager and Coaches release the Port Moody Amateur Hockey Association (PMAHA) and all Officials and Volunteers associated with the tournament from any liability for injury or accident which may be incurred by any player or team official while attending or traveling to, or from, the tournament.
The Tournament can hold a maximum of 32 teams with the number in each division yet to be determined. PMAHA reserves the right to not accept applications due to an imbalance in the division structure.
