Client Rights
Every Client participating in services with Jorgensen Counseling is entitled to basic rights:
1.The right to treatment/services that support personal rights and freedoms.
2.The right to an individualized, written treatment plan: periodic review and assessment of needs, and appropriate
revisions of the plan and/or aftercare.
3. The right to on-going participation in the planning and revision of services to be provided and the right to
reasonable explanation of all aspects of one’s own condition and treatment.
4. The right to refuse treatment.
5.The right to refuse participation in the experimentation or research unless informed, voluntary, written consent is
given; the right to appropriate protection if participating; and the right to revoke such consent.
6.The right to freedom from restraint or seclusion unless these is imminent danger to self/others and all less
restrictive means have been exhausted.
7. The right to humane treatment that affords reasonable protection from harm, appropriate privacy, and freedom
from verbal and/or physical abuse.
8.The right to confidentiality of records.
9.The right to access, upon request, to his/her own client records in accordance with state law.
10.The right to be informed, in appropriate language and terms, of their rights, including the right to legal counsel and
all other requirements of due process.
11.The right to not be subjected to remarks which ridicule or demean.
12.The right to not be exploited, or required to make public statements acknowledging gratitude to the program or
perform at public gatherings.
13.The right to assert any grievances with respect to rights, services or other grievances and to have them considered
in a fair, timely, and impartial manner; the right to appeal any client rights violations.
14.The right to a qualified advocate, to exercise/protect his/her rights.
15.The right to be informed, in advance, of charges for services.
16.The right to all available services without discrimination because of race, creed, color, sex, age, handicap, national
origin, or marital status.
17.The right to referral, as appropriate, to other providers and/or services.
18.The right to exercise his/her civil rights, including but not limited to, the right to register and vote, to
acquire/dispose of property, enter into contractual relationships, to marry and/or divorce, to hold professional,
occupational or vehicle operator’s license, unless adjudicated incompetent or finding that individual is unable to
exercise the specific rights(s).
19.The right to a smoke free environment at the Jorgensen Counseling office site.
