Immunization Requirementsforthe 2015-2016 School Year

The Missouri state law requires that all students must have documentation of all his/her immunizations. Your child will not be allowed into the classroom without proper documentation. Minimum acceptable immunizations consist of:

  • 4 doses DPT, DTaP or DT as long as the last dose is on or after the 4th birthday
  • 3 doses of Polio vaccine as long as the last dose is on or after the 4th birthday
  • 2 doses of MMR, the first given on or after the 1st birthday
  • 3+ doses of Hepatitis B (8 weeks between doses 2 and 3, 16 weeks between doses 1 and 3 and 3rd dose to be given after age 24 weeks)
  • 2 doses Varicella (Chicken Pox) first vaccine on or after the 1st birthday, minimum interval between doses is 3 months - documentation of ChickenPox disease provided by physician is an acceptable alternative to vaccine

Students who have not been fully immunized will not be in compliance with the law and will be

placed on an “Immunizations in Progress” status, provided there is a state issued Immunizations in Progress card signed by your physicianof at least one dose of eachrequired vaccine. The parent or guardian will be asked to provide an appointment date for thenext due dose of vaccine from a health care provider. This appointment date shall serve as thesuspension date if the child fails to keep the scheduled appointment. This process shall continueuntil acceptable immunization status is attained.

Change of Clothes Keeps Your Child from Missing Out

We need your help! Please send a change of clothes, to include undies/panties and socks, appropriate for the season. Kindergarten is full of milk spills, muddy knees, recess puddles and bathroom accidents. Unfortunately,we do not have the space to clothe our growing elementary student body! Please send your child’s top, bottom, undies and socks in a plastic gallon zipper seal bag labeled with his/her name. This may be kept in his/her cubby.

When to Keep Your Child at Home

Please do not send your child to school if they have any of the following:

Fever of 100 degrees or above (without medication) in the last 24 hours

Vomiting or diarrhea

Persistent cough

Unexplained rash

Pink or draining eyes

Keeping your child home until they are over their illness will not only help your child build up their own immune system so they won’t catch another illness, but will also protect the other children and staff.

Current Contact Information and Health Concerns

The Health Inventory sheet you complete will be the primary means for us to care for your child at school based on the health history and concerns you have shared. This information must be kept up-to-date.

Please contact us with phone number changes not only for parents/guardians but also the emergency contacts.

Medications at School

If it becomes necessary for a student to take any form of medication at school:

  • Parent/Guardian must bring the medication in the original container (over-the-counter) or with a prescription label to the health office (ask the pharmacist for a 2nd container for school)
  • Written parental consent must be on file in the health office – parent will sign a medication administration record upon drop off in health office
  • Students are not allowed to carry medication on the bus or in their backpack unless appropriate forms have been completed in the health office (i.e. inhaler)
  • Do not send medication with a child and do not send medication without being in a proper container.
