Directions:

·  Please type a complete response to 10 of these questions and submit Questionnaire no later than

May 26th.

·  Choose any 10 questions and provide a thorough response.

·  Your college counselor is looking for depth in your responses and a true sense of what matters to you in order to convey the value you will bring to the college you attend.

1. Write a short paragraph to introduce yourself to a college. What kind of person are you, what do you care about, what are the most important things in your life?

2. Using your StrengthsExplorer, MI Advantage or Do What You Are assessment themes, describe an example of how one of these themes is related to your thoughts and actions. If none fit, choose another theme.

3. In a competitive applicant pool, college admissions officers read the Counselor Letter of Recommendation letter looking for qualities that a student will bring to the dorms and social life, the academic classrooms and labs, and the overall college community. What would you like your counselor to emphasize in describing your potential contribution?

4. What role does religion, faith, spirituality, or God play in your life, if any? Are you active in a church or organized religion? What kind of things do you care about in that environment?

5. Is there an activity or passion for something in your life that you have inspired other students to care about? Tell that story.

6. Colleges never want the counselor to rehash a student's resume, but your activities often include a back-story not evident from the list of things you have done. Tell that back-story. What did this activity mean to you? What qualities did this activity develop in you? How has this activity changed your viewpoint or purpose?

7. What are the three words your friends would consistently use to describe you? What do they know about you that may not always be on display here at school?

8. Who are the people you look up to, and what are the qualities you admire? How do you show those same qualities?

9. Has there been a challenge in your life that has been difficult to overcome? Is there a story that you would like your counselor to tell about your ability to make a positive outcome out of a difficult situation?

10. What makes you smile? (Activities, people, events, hobbies, projects, etc.)

11. What are your favorite things to do in the past? What about now?

12. What activities make you lose track of time?

13. What makes you feel great about yourself?

14. Who inspires you most? (Anyone you know or do not know. Family, friends, authors, artists, leaders, etc.) Which qualities inspire you, in each person?

15. What are you naturally good at doing? (Skills, abilities, gifts etc.)

16. What do people typically ask you for help in?

17. If you had to teach something, what would you teach?

18. What would you regret not fully doing, being or having in your life?

19. You are now 90 years old, sitting on a rocking chair outside your porch; you can feel the spring breeze gently brushing against your face. You are blissful and happy, and are pleased with a wonderful life. Looking back at your life and all that you’ve achieved and acquired, all the relationships you’ve developed; what matters to you most? List them out.

20. What are your deepest values?

21. What were some challenges, difficulties and hardships you’ve overcome or are in the process of overcoming? How did you do it?

22. What causes do you strongly believe in? Connect with?

23. If you could get a message across to a large group of people, who would those people be? What is your message?

24. Given your talents, passions and values, how do you use these resources to serve, to help, to contribute? (to people, beings, causes organization, environment, planet, etc.)

25. Please write any additional insights about yourself that have not been addressed in the previous questions.
