TUITION ASSISTANCE/SCHOLARSHIP INSTRUCTIONS 2012-2013

(For incoming 9th grade students in Fall 2012)

Procedures & Checklist

□  Fill out “Catholic Education Foundation Application” (3 pages). SAHS uses this form for both CEF tuition assistance and any other tuition assistance, including scholarship awards, provided by SAHS. THIS FORM MUST BE SIGNED ON THE 3RD PAGE!! Please fill out this form even if you have already applied in October through your Catholic elementary school.

□  Provide copy of 2010 Tax Return (only first two pages) and 2010 (not 2011) W-2”s.

□  If applicable, provide any other supporting documentation such as disability, social security, welfare, unemployment statements etc.

□  FOR PARENTS: Write a letter to Tuition Assistance Committee on attached form explaining need and any other circumstances we should be aware of. NOT REQUIRED IF YOU ARE ONLY APPLYING FOR SCHOLARSHIP PROGRAM. See scholarship program requirements.

□  FOR STUDENTS: Write an essay on the attached form about why he/she wants to attend St Anthony High School. NOT REQUIRED IF YOU ARE ONLY APPLYING FOR SCHOLARSHIP PROGRAM. See scholarship program requirements

□  Sign the bottom of this form. DEADLINE TO RETURN JANUARY 13, 2012

Limitations, Conditions & Restrictions

I.  The amount of tuition assistance will be based on verified financial need. Ordinarily no more than 40% of the yearly tuition will be granted.

II.  The finance committee will make the final decision regarding the amount of financial assistance given.

III.  Student must maintain a 2.0 GPA to continue receiving aid.

IV.  The amount granted will be applied to the yearly tuition. All families must still pay regular yearly fees. The remainder of the tuition is to be paid in regular monthly installments through the SMART Tuition program.

V.  A new tuition assistance application must be submitted each year.

VI.  Any changes in income must be reported to SAHS. Large purchases must be justified for assistance to continue.

VII.  The tuition assistance committee is under no obligation to review your application for assistance if application is incomplete, missing information or signatures, or lacking adequate proof of income.

Please write a letter to the Tuition Assistance Committee explaining why you wish to attend SAHS. If you are applying for a scholarship only, you DO NOT need to provide this letter.
