Indemnity Waiver and Release of Liability

I, ______, am the parent /volunteer, who wishes to participate as a volunteer of Beagles & Buddies.

In consideration for being permitted by Beagles & Buddies to participate as a volunteer, I hereby agree that I will assume full responsibility for the death, personal injury or property damage suffered or sustained me as a result of or in connection with my participation as a volunteer.

I agree, therefore, to indemnify Beagles & Buddies from and against all liability, claims, demands, damages, judgments and costs for or arising from the death or personal injury of myself or damage to the my property suffered or sustained by me as a direct or indirect result of the my participation as a volunteer; even if such death, personal injury or property damage arises out of the negligence of Beagles & Buddies (or any of its employees, volunteers or officers).

I further agree that I will not make any claim against, sue, attach the property of, or prosecute Beagles & Buddies (or any of its employees, volunteers or officers) for any death, personal injury or property damage, whatever the cause or place of the event giving rise to the claim, which I may may suffer or sustain as a result of or in a connection with my participation as a volunteer.

In addition, I hereby release and discharge Beagles & Buddies (and its employees, volunteers or officers) from all actions, claims or demands that I now have or may hereafter have for any death, personal injury or property damage arising out of or in connection with my participation as a volunteer.

This release of liability is intended to discharge in advance, Beagles & Buddies (and its employees, volunteers or officers) from and against any and all liability to me or to any person claiming under, through or on behalf of either of us arising out of or connected in any way with my participation as a volunteer; even though that liability may arise out of negligence on the part of Beagles & Buddies (and its employees, volunteers or officers).

I further understand that the behavior of domestic animals is sometimes unpredictable and that some domestic animals are capable of inflicting serious personal injury or death, as well as extensive property damage.

Knowing the risks of handling domestic animals; nevertheless, I hereby agree to assume those risks and to release, indemnify and hold harmless Beagles & Buddies and the persons mentioned above who might otherwise be liable to me or my heirs or assigns for damages.

It is understood and agreed that this Indemnity, Waiver and Release of Liability is intended to be binding on the me and my heirs, distributes, legal representatives or assigns.

I represent that I am over eighteen years of age.

I HAVE READ THIS AGREEMENT CAREFULLY AND FULLY UNDERSTAND ITS CONTENTS.

I AM AWARE THAT THIS IS AN INDEMNITY WAIVER AND RELEASE OF LIABILITY AND

A CONTRACT BETWEEN BEAGLES & BUDDIES AND MYSELF, AND I SIGN IT OF MY

OWN FREE WILL.

Confidentiality Agreement

In the pursuit of its objectives, Beagles & Buddies finds it useful to use the services of volunteers, contract, employees, and other outside regular staff. Not infrequently, these individuals will be permitted to use and

see books, records, computer data and programs, and other information deemed by B&B to be its property and, therefore, proprietary. This information includes, without limiting the generality of the foregoing, donor and

membership information, volunteer identification information, planning documentation, computer programs, operational financial background information and current financial material.

In executing this document I recognize the proprietary nature of this property. I promise that I shall not convert or use this proprietary property in any manner or for any purpose not authorized by Beagles & Buddies.
