PROCEDURES FOR CONVERTING RAW SCORE TO NORMATIVE SCORE
  1. Add the total of the employee’s raw score. Multiply the Weighting Factor of 1.67 times the RAW SCORE. This will produce the NORMATIVE SCORE. Enter this number beside NORMATIVE SCORE below.
  1. NOTE:ANY score of 2 or less on any of the factors above should trigger a written work improvement plan with a three to six month re-evaluation. A score of 2 or less on more than two of the above factors may be terms for disciplinary action, up to and including dismissal.
  1. All evaluations are due by the first of the month prior to the month in which the employee was hired. For example, the annual evaluation for an employee hired on January 12, 2009, would be due December 1 2009. Late evaluations beyond this cut off date may result in a delay in any pay increase for the employee.
