A little background on the situation I used to produce by KT plan:

I am certainly in over my head, but am in nonetheless. I have mentioned in my forum posts that I am a member of a task force working to bring some solutions to the table for some pretty nasty troubles in the state of Arkansas. I love the group with whom I am working! Unfortunately, I hate the situation we are tasked with addressing. Here’s the history. Arkansas is a bit of an anomaly in the way we have access to children who need services. But, this has caused problems with some providers practicing fraudulently. Though these providers have been reported by other providers and can be selected out of statewide data as “over-prescribers,” these practices have really put the spotlight on therapy services. These issues, in combination with PCMH and even proposed managed care issues in our state have really prompted a great deal of distrust between therapists, physicians, and legislators. Knowledge is power! We are going to have to educate policy makers regarding best practices, assessment, dosing, and a number of other key components of our profession. Currently, my focus is on dosing. The legislature would like us to provide dosing standards. For example, if a child scores this, then they receive this. I, along with many other members of our team, feel that this negates a therapists ability for clinical decision making and reduces our therapy prescription to just a numbers game. With that said, I am scouring the literature for information on dosing to help shed some light on how dosing decisions are made, why they are made, what we know, what we do not know, and how we can move forward.
