FIM# 612924 – Appalachia
RWC: Employee was replacing track pads on an excavator with cleated track pads. One of the pads was stuck to the machine. The mechanic placed left hand on the bottom of the stuck track pad near the end while using a pry bar in an attempt to free the pad. When the pad came free it fell pinching the employees hand between the freed track pad and the bottom track. Employee then called company HSE who then notified Shell. Employee was taken to a local medical facility for treatment. / 3 / 3 / 10

Last Motor Vehicle Incident

13 Jan 2011:
FIM# 610208 – Appalachia
MVI: EE was travelling to hotel when a vehicle ran a red light and hit the vehicle two lanes to the left; both of those vehicles slid across EE's lane and subsequently clipped the very rear corner of the vehicle in the middle. Minimal damage to vehicle EE was in: there were no injuries.
Police were called and a full report has been completed on the scene. Initial driver has cited and is being taken for tox screen. / 1 / 1 / 18
Key Message from Bob Jefferis:
It is with a heavy heart that I must report that a British Columbia subcontractor passed away in hospital on February 17. The employee was last at work on February 10 unloading mats from the back of a flatbed truck. The fatality continues to be under investigation by WorkSafeBC, Shell, and Contractors. Regardless of the outcome of the investigations, this tragic event is a painful reminder that any task, no matter how routine it may seem or how many times it has been performed safely before, has the potential to cause injury. Vigilance and caution should be our constant companions especially as we go about our daily routine tasks. Our thoughts and prayers extend to the individual, and family and friends impacted by this event.
I want to focus this note on the actions we as leaders can take to bring our heartfelt passion for an injury free workplace to the field. There is one best way to convey this message: we must personally show up in the field, in the workplace, and observe, engage, and listen to our staff and our contractors on safety issues as they do their work. Our presence on site, with dedicated time to safety activities, is the most powerful signal we can send on the priority we place on the safety of our workers.
I and my leadership team have made the commitment to get out to the field at least once per month and spending quality time observing and engaging our workforce on safety. We also commit to providing written feedback to our hosts and each other following every visit. We will be tracking our achievement of this commitment on a monthly basis. You may likely be called upon to host one or more of these visits during the year. I thank you in advance for working with my leaders and I to get to the field and dedicate time to safety activities with our workers.
Likewise, I expect every line leader to commit to a field safety visit frequency, appropriate for their role, with their supervisor.
As the Chair of the Shell United Way Campaign in Calgary last year, I learned the power of many small contributions adding up to a significant positive impact across our community. In much the same way, I believe the accumulated time that we together dedicate to safety engagements in the workplace will have a positive impact across our community of workers.
