The overall purpose of this survey

This survey is designed to get information from you regarding your experiences at work. The data gathered using this tool will enable us to identify how well the health care organization you work for is meeting your needs including areas that may need improvement. Neither your supervisor nor anyone else will know how you responded to this survey. The results we get from this survey will be reported in general terms and will not identify individuals.
