JOB DESCRIPTION:

Provides relevant nursing care within an innovative Team Care Medicine Model, in an ambulatory setting. Nursing care is provided under the guidance and supervision of a registered nurse and clinical leadership. Responsibilities include collection of clinical data, care coordination, administration of medications and treatments, and documentation of care provided.

Scheduling: Intermittent Saturday hours every 20-25 weeks along with assigned medical provider. No call.

Combines nursing, care coordination, immunizations administration, lab processing, patient education, NO phlebotomy

1)  Nursing skills – within scope of LPN – patient rooming, onsite lab completion, basic testing, nebulizer treatments, injectable medications (within scope of practice), immunizations

2)  Care coordination (site specific)– referral/test tracking, patient education and self-management support, and immunization admin to be completed per protocols generated by RN manager built within the scope of practice for LPN in NV

3)  Administrative duties

  1. Potential for front office and scheduling skills with the Practice Manager software
  2. Community outreach events
  3. Community events as representative of CHA
  4. Public speaking engagements with medical provider and administrator

4)  Time allocations

  1. Clinical days
  2. Potentially requires front office skill in check-in and discharge
  3. Minimum of 2 patients per hour
  4. Communications liaison for care team
  5. Immunizations
  6. Lab processing
  7. Other nursing tasks assigned
  8. Non-clinical days
  9. Care coordination (based upon care coordination protocols – referrals/test tracking, communication liaison, patient engagement, etc)

5. Ensure all patient charts and related documentation are maintained current and relevant at all time.

7. Demonstrate competency in practice and knowledge of current standards of practice. Maintains credentials and performs current nursing practice expectations within established guidelines.

9. Develops and maintains favorable internal relationships, partnerships with co-workers, including health center managers, nursing staff, medical assistant staff, medical providers, care coordinators, and business office staff.

PRIMARY TASKS & DUTIES

1. Provide direct care to patients as assigned and qualified to perform.

2. Assess, review, and develop nursing plans to ensure maximum patient outcomes

3. Provide counsel, support, advice, and support to patients and their families

4. Complete all patient charting, and ensure all patient records are current and complete

5. Attend meetings, patient conferences, planning sessions, related to quality improvement/assurance, patient care, and other related topics within the health center and CHA

6. Attend seminars and maintain all licensure requirements for continuing education and best practices

7. Participates in quality strategies to evaluate compliance with standards and to identify opportunities to improve patient outcomes.

POSITION REQUIREMENTS

Education

• Graduation from an accredited practical or vocational nursing program.

·  Current and Unrestricted LPN license from the Nevada Board of Nursing

·  Current BLS - CPR certification

Experience

• Three years of LPN practice experience; family medicine practice or other primary care setting preferred

·  Demonstrated knowledge and abilities of nursing skills

·  Clerical/administrative experience a plus

·  Demonstrated success in customer service or working with the general public, preferably in a medical care facility.

• Demonstrated success in managing difficult customer situations

• Significant knowledge of medical practices and procedures in a primary care environment.

• Demonstrated success and experience with direct patient services and the proper knowledge and use of medical devices and equipment.

·  Spanish language skill preferred

Physical/ Environmental

• Occasional lifting of 40 – 50 lbs and pushing of 5-20 lbs

• Sitting, standing and walking approximately 80% of the time

• A medium to high level of manual dexterity required

• Bending and reaching approximately 20% of the time

Education and Licensure:

  1. Graduate with a LPN designation.
  2. Current CPR certification
  3. Current and Unrestricted LPN license from the NV Board of Nursing

Experience:

I.  Current work experience in a family medicine practice or other primary care setting preferred

II.  Demonstrated knowledge and abilities of nursing skills

III.  Clerical/administrative experience a plus

IV.  Spanish language skill preferred
