ABOUT NEW HAVEN FARMS:

New Haven Farms’ nonprofit charitable mission is to promote health and community development

through urban agriculture. We transform vacant, contaminated urban land into vibrant andproductive organic farms. Our Farm Manager and volunteer corps cultivate urban farms, and our harvest is made accessible to low­income families who struggle with diet­related chronic diseases

and food insecurity. Farm members receive produce baskets combined with farm education, cooking demonstrations, and nutrition education on a weekly basis. For more information about NewHaven Farms, visit newhavenfarms.org.

POSITION SUMMARY AND RESPONSIBILITIES:

Community Liaisonsprovide critical support to New Haven Farms’Farm-Based Wellness program participants. They advocate for program participants, connect participants with each other, provide encouragement and logistical aid, collect weekly surveys and data, and ensure that each participant comes away from the program having improved the quality of their own lives. Specific responsibilities include but are not limited to:

  • Manage program participant recruitment and referral process beginning in April, including interfacing with health institution staff and administration, referred patients and patient families, and New Haven Farms staff and volunteers.
  • Create and utilize program participant recruitment and referral materials and systems.
  • Attend Farm-Based Wellness Program sessionsfrom June-October. Assist with setup, cleanup, translation, and data collection at each session.
  • Oversee team of Community Liaison volunteer and interns.
  • Call program participants to ensure their attendance at program sessions, and document all call outcomes.
  • Conduct data collection on a weekly basis, and follow up with those who missed program sessions.
  • Provide general logistical support to program participants.
  • Document attendance.
  • Attend New Haven Farms staff/volunteer/intern meetings, which take place both in person and via phone/skype. Contribute updates and agenda items for these meetings.
  • Write best-practices manual for Community Liaison position, and complete all follow-up activities and data collection with health institution staff and administration, referred patients and patient families, and New Haven Farms staff and volunteers.

QUALIFICATIONS:

  • Fluency in both English & Spanish, ability to interpretin real time.
  • Ability to assist in executing community-based events.
  • Ability to work with people of various ages and skill levels.
  • Ability to use basic computer software –email/word/excel primarily.
  • Strong attention to detail and record-keeping.
