For many years, churches and individuals from America have been going overseas to assist missionaries and national churches with construction and ministry teams. This has had a tremendous impact with both those sending and those receiving.
We have been blessed of the Lord, having had so few accidents with minor injuries. However, due to an attempt that has been made to litigate a claim against us on a very questionable case, we are requiring that all team members sign an Assumption of Risk and Insurance Agreement form. The Assemblies of God World Missions and the MAPS Construction office ask for 100% cooperation, without exception, of all teams and individuals who are planning an overseas mission journey. As a requirement of the AGWM Executive Committee, no team or individual is authorized to depart the states until these documents have been mailed to our office in Springfield, MO.
All important documents are enclosed. The insurance is with Guarantee Trust Life Insurance Company (GTL). Effective September 1, 2006, the Executive Committee of the Assemblies of God World Missions has made it mandatory for all teams registering through the MAPS Construction office to purchase the GTL overseas insurance. The cost of the insurance coverage is $2.40 per day, per person (travel days need to be included in the total). Only registered and approved teams are allowed to buy the insurance provided through the Assemblies of God World Missions. If you have not called to register your team with the AGWM MAPS Construction office two months prior to the team’s departure, you can do so by calling 417-862-2781 ext. 2067.
Originals of the Following to be Mailed Two Weeks Before Team’s Departure:
1) a completed Insurance List Report form giving the names of team members going on the trip and information on the team’s missions trip (to be filled out once by Team Leader or Team Administrator)
2) a completed Assumption of Risk and Insurance Agreement form for each team member (these originals must be mailed, not faxed)
3) a completed GTL Beneficiary Designation Form (these originals must be mailed, not faxed)
4) a completed MAPS Construction Code of Conduct Form (these originals must be mailed, not faxed)
5) a check for the total insurance premium. (include dates of travel in total)
*Please make check payable to AGWM Admin Medical Insurance and place insurance account # 5369541 cl (08) on the memo line of check.
The originals of the five items listed above need to be in the AGWM Insurance office two weeks prior to the team’s departure, allowing adequate time to process the forms, thereby, granting the team clearance for the trip.
